改良鼻饲管置入法在急性缺血性脑卒中伴吞咽困难病人中的应用

摘　　要：目的:评估改良鼻饲管置入法在急性缺血性脑卒中伴吞咽困难病人中的应用有效性及安全性。方法:选取2017年8月—2019年1月住院的80例急性缺血性脑卒中伴吞咽困难的病人为研究对象,采用随机数字表法将其分为观察组和对照组各40例,观察组采用半卧位改良鼻饲管置入法,对照组采用传统鼻饲管置入法,观察、记录两组病人的一次插管成功率、操作用时、并发症(呕吐、呛咳、黏膜出血)发生率、置管前后心率及收缩压变化。结果:观察组病人一次插管成功率明显高于对照组,操作用时明显短于对照组,并发症发生率明显低于对照组,置管后心率及收缩压明显低于对照组,两组比较差异均有统计学意义(P<0.05)。结论:改良鼻饲管置入法较传统法一次插管成功率更高、操作更快、并发症更少、安全性更高。Objective:To evaluate the efficacy and safety of modified implant method of the nasogastric tube in patients with acute ischemic stroke complicated with dysphagia.Methods:A total of 80 patients with acute ischemic stroke complicated with dysphagia who were hospitalized from August 2017 to January 2019 were selected as the research subjects.They were divided into observation group and control group by random number table method,40 patients in each group.The patients in observation group were treated with semi-decubitus modified nasogastric tube implantation method,while the patients in control group were treated with traditional nasogastric tube implantation method.The disposable success rate of intubation,operation time,incidence of complications(vomiting,choking,mucosal bleeding),heart rate and systolic blood pressure before and after intubation were observed and recorded in the two groups.Results:The disposable success rate of intubation in the observation group was significantly higher than that in the control group,the operation time was significantly shorter than that in the control group,the incidence of complications was significantly lower than that in the control group,the heart rate and systolic blood pressure after intubation were significantly lower than that in the control group,the differences were statistically significant(P<0.05).Conclusions:Compared with the traditional method,the modified implant method of the nasogastric tube had higher disposable success rate of intubation,faster operation,fewer complications and higher safety.
