摘 要:目的:探讨慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭患者进行无创正压通气(NPPV)治疗时滴定呼气灵敏度对患者主观舒适度及治疗效果的影响。方法:入组对象为2018年10月~2019年9月本院收治的AECOPD合并呼吸衰竭患者共78例,随机分为对照组和观察组。两组治疗时均使用无创呼吸机进行正压通气治疗。对照组患者实施固定呼气灵敏度设置,观察组患者实施滴定呼气灵敏度设置。对比两组患者主观舒适度以及治疗效果。结果:主观舒适度方面,观察组患者对呼吸通畅、呼吸机送气时间、呼吸切换时间以及呼吸频率的满意比例高于对照组(P<0.05)。动脉血气指标方面,两组患者pH无统计学差异(P>0.05),观察组PaCO2和PaO2指标优于对照组(P<0.05)。结论:在AECOPD合并呼吸衰竭患者进行NPPV辅助治疗时,采用滴定呼气灵敏度,可提高患者治疗舒适度,改善其通气功能。Objective:Explore the effect of titration expiratory sensitivity on subjective comfort and therapeutic effect in patients with AECOPD and respiratory failure treated with NPPV.Methods:The enrolled subjects were 78 AECOPD patients with respiratory failure admitted to our hospital from October 2018 to September 2019.They were randomly divided into control group and observation group.Both groups used non-invasive ventilator for positive pressure ventilation during treatment.Patients in the control group implemented a fixed expiratory sensitivity setting,and patients in the observation group implemented a titrated expiratory sensitivity setting.Compare the subjective comfort and treatment effect of the two groups of patients.Results:In terms of subjective comfort with study group,the ventilation time,ventilation switching time and respiratory frequency comfort were higher than those in the control group(P<0.05).There was no significant difference in pH between the two groups(P>0.05),the indexes of PaCO2 and PaO2 in the study group were better than those in the control group(P<0.05).Conclusion:When the patients with AECOPD and respiratory failure were treated with NPPV,the sensitivity of titration breath could improve the comfort of treatment,improve ventilation.
