摘 要:目的:对比采用阈值触发法与小剂量测试法在CT血管造影(CT angiorgraphy,CTA)冠状动脉峰值采集中的效果差异。方法:回顾性分析212例冠状动脉CTA成像患者的影像资料,采用阈值触发法扫描的患者109例(作为阈值触发法组),采用小剂量测试法扫描的患者103例(作为小剂量测试法组),以升主动脉根部(左冠开口层面)CT值作为冠状动脉峰值。采用Wilcoxon秩和检验比较2组患者在冠状动脉采集时的心率变化、图像质量评分、CT容积剂量指数(CT dose index volume,CTDIvol)、剂量长度乘积(dose length product,DLP)的差异;采用两独立样本均数t检验比较2组患者心率及升主动脉根部(左冠开口层面)CT值的差异。结果:2组患者在心率及心率变化、图像质量评分对比上差异不具有统计学意义(P均>0.05)。阈值触发法组的升主动脉根部(左冠开口层面)CT值、CTDIvol及DLP等参数优于小剂量测试法组,差异具有统计学意义(P<0.05)。结论:阈值触发法相比小剂量测试法在冠状动脉CTA成像中辐射剂量低,且更易获取较高的冠状动脉峰值,可通过进一步减少对比剂用量,达到冠状动脉成像预期效果。Objective To compare the effects of bolus tracking technique and test bolus technique in coronary artery CTA peak collection.Methods The imaging data of 212 patients with coronary artery CTA imaging were retrospectively analyzed,of whom 109 patients scanned with bolus tracking technique were enrolled into a bolus tracking technique group and 103 cases with test bolus technique were included into a test bolus technique group.The CT value of the ascending aortic root at the level of the left coronary opening was adopted as the peak coronary artery value.Wilcoxon rank sum test was used to compare the differences in heart rate variation,image quality score,CT dose index volume(CTDIvol)and dose length product(DLP)between the two groups during coronary artery acquisition.The differences in heart rates and CT values at the ascending aortic root at the level of the left coronary opening were compared between the two groups.Results The differences of the two groups were not significant in heart rate,heart rate variability and image quality score(P>0.05),while the patients in the bolus tracking technique group had statistically higher CT values,CTDIvol and DLP in the ascending aortic root at the level of the left coronary opening than the ones in the test bolus technique group(P<0.05).Conclusion Bolus tracking technique has advantages in low radiation dose and low difficulty in obtaining high peak coronary artery value when compared with test bolus technique,which can achieve the expected effect of coronary artery imaging by reducing the amount of contrast agent.
