摘　　要：目的探讨并对比不同精索静脉曲张手术治疗精索静脉曲张的临床效果。方法选择2017年1月—2019年12月在医院治疗的50例行精索静脉曲张手术的患者作为研究对象,同时根据患者术式不同将入选对象分为观察组(26例)及对照组(24例)。对照组给予经腹膜后精索静脉高位结扎术治疗,观察组给予改良经腹股沟下显微精索静脉结扎术治疗,对两组总体有效率、并发症发生率以及术后6个月精液质量进行对比。结果观察组的总体有效率、并发症发生率分别为96.16%、3.84%,高于对照组的总体有效率(75.00%)和并发症发生率(33.33%),差异有统计学意义,P<0.05,观察组术后6个月的观察组的精子密度为(176.27±17.54)×10^(6)/mL,(A+B)级活动力精子为(60.72±6.14)%,精子畸形率为(19.42±1.84)%,对照组术后6个月的精子密度、(A+B)级活动力精子以及精子畸形率分别为(132.16±13.07)×10^(6)/mL、(45.61±4.47)%、(28.41±2.76)%,观察组的精子密度、(A+B)级活动力精子、精子畸形率均优于对照组,差异有统计学意义,P<0.05,t=11.045、10.897、14.844。结论精索静脉曲张采取改良经腹股沟下显微精索静脉结扎术治疗,能够改善精子质量,减少术后并发症的发生,提升疗效。Objective To evaluate and compare the clinical effect of different varicocele surgery on varicocele.Methods From January 2017 to December 2019,50 patients with varicocele surgery were selected as the study subjects,and the subjects were divided into observation group(26 cases)and control group(24 cases)according to the different surgical procedures.The control group was treated with high-position ligation of retroperitoneum spermatic vein,and the observation group was treated with modified transoccuperic microcoronary ligation.Results View The overall effective rate and complication rate of the observation group were 96.16%and 3.84%,respectively,higher than that of the control group(75.00%)and the incidence of complications(33.33%),the difference was significant,the difference was statistically significant,P<0.05,the sperm density of the observation group was(176.27±17.54)×10^(6)/mL,the activity of grade A(60.72±6.14),the abnormality rate of sperm(19.42±1.84%),and the sperm density of 6 months after operation in the control group degree,(A+B)level motility sperm and sperm deformity rate were(132.16±13.07)×10^(6)/mL,(45.61±4.47)%,(28.41±2.76)%.sperm density,(A+B)level motility sperm and sperm deformity rate in the observation group were better than those in the control group.the difference was significant and statistically significant,P<0.05,t=11.045,10.897,14.844.Conclusion The treatment of varicocele with modified transoccupying microcoronary ligation can improve the quality of sperm,reduce the incidence of postoperative complications and improve the curative effect.
