四种不同检查方法在肺癌诊断中的应用价值

摘　　要：目的研究纤维支气管镜刷片(简称刷片)细胞学、支气管肺泡灌洗液(简称灌洗液)细胞学、经支气管镜肺活检术(简称活检)、经皮肺穿刺(简称穿刺)在肺癌诊断中的价值。方法回顾性分析161例原发性肺癌患者的刷片、灌洗液、活检、穿刺的病理检查结果,通过比较4种不同方法的诊断阳性率来评价其对肺癌的诊断价值。结果161例肺癌患者的刷片、灌洗液、活检、穿刺的诊断阳性率分别为44.10%(71/161)、32.30%(52/161)、87.25%(89/102)、97.44%(76/78);刷片的阳性率与刷片+灌洗液的阳性率比较,差异无统计学意义;活检+刷片+灌洗液的阳性率显著高于刷片+灌洗液;穿刺+刷片+灌洗液的阳性率显著高于刷片+灌洗液;4种方法联合检查阳性率为100%(19/19)。结论通过刷片、灌洗液、肺活检、肺穿刺多种方法的联合检查可提高肺癌检出率,为临床提供更多的诊断依据。Objective To study the value of fiberoptic bronchoscopic Brushing Cells(brush for short),bronchoalveolar lavage fluid cytology(lavage for short),transbronchoscopic lung biopsy(biopsy for short)and percutaneous lung puncture(puncture for short)in the diagnosis of lung cancer.Methods The pathological examination results of brush,lavage,biopsy and puncture in 161 patients with primary lung cancer were analyzed retrospectively.The diagnostic value of four different methods was evaluated by comparing the positive rates of diagnosis.Results The positive rates of brush,lavage,biopsy and puncture in 161 cases lung cancer were 44.10%(71/161),32.30%(52/161),87.25%(89/102)and 97.44%(76/78),respectively.There was no significant difference between the positive rates of brush group and brush+lavage group.The positive rate of biopsy+brush+lavage group was significantly higher than that of brush+lavage group.The positive rate of puncture+brush+lavage group was markedly higher than that of brush+lavage group.The total positive rate of the four methods was 100%(19/19).Conclusion The combined examination of brush cytology,lavage fluid cytology,lung biopsy and lung puncture can improve the positive detection rate of lung cancer,and provide more diagnostic basis for clinical practice.
