摘  要:股骨头坏死是骨外科常见的难治性疾病,其机制仍有待研究。目前为止,医源性糖皮质激素是非创伤性股骨头坏死的主要原因。激素的长期使用可导致股骨头骨细胞凋亡、血液循环障碍所致缺血缺氧,最终导致股骨头塌陷。激素性股骨头坏死的发生发展与骨组织中细胞直接接触和其间接分泌的细胞因子调控相关。本文综述了骨组织中成骨细胞分泌的核因子κB受体结合配体,骨保护素,骨碱性磷酸酶,骨细胞表达硬化素,破骨细胞分泌骨形态发生蛋白2等因子在SONFH的研究进展,骨源性细胞因子在SONFH中扮演重要作用。Avascular necrosis of the femoral head is a common refractory disease in orthopedic surgery,whose mechanism remains to be studied.So far,iatrogenic glucocorticoids are the main cause of non-traumatic necrosis of the femoral head.Long term treatment of glucocorticoids leads to osteocyte apoptosis of femoral head,ischemia and hypoxia caused by blood circulation disorder,and eventually leads to collapse of femoral head.The occurrence and development of steroid induced osteonecrosis of the femoral head are related to the direct contact of cells in bone tissue and the regulation of cytokines secreted indirectly.We reviewed the research progress of Nuclear factor kappa B receptor binding ligand,RANKL,Osteoprotegerin,OPG and Bone alkaline phosphatase,BAP secreted by osteoblasts,sclerotin(SOST)produced by osteocytes,and Bone morphogenetic protein-2,BMP-2secreted by osteoclasts in steroid induced osteonecrosis of the femoral head.Bone derived cytokines play an important role in steroid induced osteonecrosis of the femoral head.
