背根神经节低温等离子消融术治疗带状疱疹后神经痛的效果分析

摘　　要：目的探讨背根神经节低温等离子消融术治疗带状疱疹后神经痛的效果。方法选取2018年3月至2019年12月航天中心医院收治的84例带状疱疹后神经痛患者为研究对象,按照随机数字表法分为对照组和观察组,各42例。对照组应用常规药物治疗,观察组应用背根神经节低温等离子消融术治疗。比较2组患者治疗前、治疗10 d和1、2个月后的疼痛程度和睡眠质量,观察不良反应发生情况。结果观察组治疗10 d和1、2个月后的麦吉尔疼痛问卷简表情感类[(3.1±0.8)分比(5.2±1.2)分、(2.2±0.7)分比(4.1±0.8)分、(1.8±0.4)分比(3.6±0.8)分]和感觉类评分[(6.4±1.0)分比(12.5±1.4)分、(5.0±0.5)分比(9.4±1.0)分、(4.2±0.8)分比(8.2±1.3)分]、视觉模拟量表评分[(4.5±0.8)分比(6.0±1.1)分、(3.8±0.8)分比(5.1±1.0)分、(3.0±0.5)分比(4.2±0.8)分]及匹兹堡睡眠质量指数表评分[(7.9±1.8)分比(10.3±2.1)分、(6.5±1.0)分比(8.7±1.1)分、(5.0±1.0)分比(6.2±1.1)分]均低于对照组,差异均有统计学意义(均P<0.05)。观察组未发生不良反应,对照组发生头晕3例(7.1%)、嗜睡5例(11.9%),但均可耐受。结论背根神经节低温等离子消融术治疗带状疱疹后神经痛较临床常规药物更为有效,能够降低患者疼痛程度,改善患者睡眠质量,且安全性较好。Objective To investigate the therapeutic effect of low temperature plasma ablation of dorsal root ganglion on postherpetic neuralgia.Methods From March 2018 to December 2019,84 patients with postherpetic neuralgia admitted to Aerospace Center Hospital were selected.Patients were randomly divided into control group and observation group,with 42 cases in each group.The control group was treated with conventional drugs,and the observation group was treated with low temperature plasma ablation of dorsal root ganglion.The pain degree and sleep quality of the two groups before treatment and 10 d,1 and 2 months after treatment were compared,and the occurrence of adverse reactions was observed.Results After 10 d,1 and 2 months of treatment,the scores of emotion and sensation of McGill Pain Questionnaire,visual analogue scale and Pittsburgh sleep quality index in the observation group were lower than those in the control group[(3.1±0.8)vs(5.2±1.2),(2.2±0.7)vs(4.1±0.8),(1.8±0.4)vs(3.6±0.8);(6.4±1.0)vs(12.5±1.4),(5.0±0.5)vs(9.4±1.0),(4.2±0.8)vs(8.2±1.3);(4.5±0.8)vs(6.0±1.1),(3.8±0.8)vs(5.1±1.0),(3.0±0.5)vs(4.2±0.8);(7.9±1.8)vs(10.3±2.1),(6.5±1.0)vs(8.7±1.1),(5.0±1.0)vs(6.2±1.1)](all P<0.05).There was no adverse reaction in the observation group.There were 3 cases of dizziness(7.1%)and 5 cases of lethargy(11.9%)in the control group,and they were tolerable.Conclusions Low temperature plasma ablation of dorsal root ganglion in the treatment of postherpetic neuralgia is more effective than conventional clinical drugs.It can reduce the degree of pain and improve the sleep quality of patients with better safety.
