西药联合通督调神针刺治疗后循环TIA眩晕30例临床观察

摘　　要：目的:观察通督调神针刺联合阿司匹林治疗后循环短暂脑缺血发作(TIA)眩晕的临床效果。方法:将60例后循环TIA眩晕患者随机分为对照组和观察组各30例。对照组在脑病科常规治疗基础上接受口服阿司匹林肠溶片治疗;观察组在对照组基础上联合通督调神针刺治疗。比较两组临床疗效,观察治疗前后中风先兆症状评分、眩晕评估评分量表(DARS)、脑血流动力学变化情况。结果:观察组总有效率为96.67%,高于对照组的73.33%,差异有统计学意义(P<0.05);相较同组治疗前,两组的中风先兆症状评分、DARS评分均降低,脑血流速度均增快(P<0.05),且观察组上述评分降低更多,脑血流速度增快显著(P<0.05)。结论:通督调神针刺联合阿司匹林治疗后循环TIA眩晕有效,其作用机制可能与改善患者的脑血流速度有关。Objective To investigate the clinical effect of Tong Du Tiao Shen acupuncture combined with aspirin in the treatment of vertigo after transient ischemic attack(TIA).Methods 60 cases of posterior circulation TIA vertigo patients were randomly divided into control group and observation group,30 cases each.The control group received oral aspirin enteric-coated tablets on the basis of routine treatment in the department of encephalopathy,while the observation group received acupuncture combined with Tongdu Regulating God on the basis of the control group.The clinical efficacy of the two groups was compared,and the changes of aura symptom score,vertigo assessment scale(DARS)and cerebral hemodynamics were observed before and after treatment.Results The total effective rate in the observation group was 96.67%,higher than 73.33%in the control group,the difference was statistically significant(P<0.05).Compared with the same group before treatment,both the observation group and the control group had lower preemptive stroke symptom scores,DARS scores,and increased cerebral blood flow velocity(P<0.05).Moreover,the above scores were more decreased in the observation group,and the increased cerebral blood flow velocity was significant(P<0.05).Conclusion The efficacy of Tong Du Tiao Shen acupuncture combined with aspirin in the treatment of circulatory TIA vertigo may be related to the improvement of cerebral blood flow velocity in patients.
