摘　　要：目的:探讨出院警告工具在预防永久性肠造口患者非计划性再入院中的应用效果。方法:将210例永久性肠造口患者随机分为干预组和对照组各105例;对照组在患者出院后实施常规出院健康教育,干预组在对照组基础上指导患者应用出院警告工具;比较两组出院后6个月内患者非计划性再入院发生率,并发症发生率,自我效能[采用造口自我效能量表(SSES)]。结果:干预组造口黏膜出血、周围皮肤并发症、粪水性皮炎发生率低于对照组(P<0.01),患者非计划性再入院发生率低于对照组(P<0.01),SSES评分高于对照组(P<0.01,P<0.05)。结论:应用出院警告工具可降低永久性肠造口患者出院后6个月内非计划性再入院发生率,减少并发症的发生,提高患者造口自我护理效能。Objective:To explore the application effect of discharge warning tools in unplanned readmission of permanent enterostomy patients.Methods:A total of 210 patients with permanent enterostomy were randomly divided into the intervention group and the control group,each with 105 cases.Patients in the control group were given routine hospital discharge health education after discharge,and patients in the intervention group were instructed to apply discharge warning tools on this basis,and comparison was made on the incidence of unplanned readmission,the incidemnce of complications,and self-efficacy[with colostomy self-efficacy scale(SSES)]between the two groups within 6 months after discharge.Results:The incidence of colostomy mucosal bleeding,the peripheral skin complications,dung dermatitis was lower in the intervention group than that in the control group(P<0.01).The incidence of unplanned readmission in the intervention group was lower than that in the control group(P<0.01).And the SSES scores in the intervention group were higher than those in the control group(P<0.01,P<0.05).Conclusion:Discharge warning tools can reduce the incidence of unplanned readmission in patients 6 months after discharge with permanent enterostomy,reduce the incidence of complications,and increase the efficiency of the colostomy patients self care.
