摘 要:目的探讨痛泻要方对肠易激综合征(irritable bowel syndrome,IBS)模型大鼠结肠动力影响的研究。方法建立IBS模型,进行实验分组,将大鼠随机分为正常组、模型组、得舒特组、痛泻要方组(n=8)。相应指标检测:(1)结肠转运功能的测定;(2)小肠推进率的检测;(3)Cajal间质细胞(interstitial cells of Cajal,ICC)移除前后结肠平滑肌条收缩性的评估;(4)Elisa法检测血浆及远端结肠C-kit、M-SCF、S-SCF的表达;(5)检测结肠组织C-kit、M-SCF蛋白的表达量。结果与正常组相比较,模型组、得舒特组、痛泻要方组大鼠的玻璃小球排出时间明显缩短(P<0.01)和小肠推进率明显升高(P<0.01);与模型组相比较,得舒特组、痛泻要方组的排出时间明显延长(P<0.01)和小肠推进率明显降低(P<0.01);得舒特组和痛泻要方组相比较无统计学意义(P>0.05)。相对于正常组,模型组大鼠结肠平滑肌收缩曲线的降低率明显升高,但是在得舒特组和痛泻要方组得以恢复。Elisa和Western blot中得舒特组和痛泻要方组显著降低了C-kit、S-SCF、M-SCF的表达水平。结论痛泻要方在治疗IBS中,通过调节C-kit/SCF信号通路,进而改变结肠平滑肌收缩,降低小肠推进率,改善肠易激综合征患者的症状。Objective To investigate the effect of Tongxie Decoction on intestinal motility in IBS rats. Methods Firstly, IBS model was established and the rats were randomly divided into normal group, model group, pinaverium bromide group and main prescription group. The corresponding indicators were derected:(1)determination of colonic transport function;(2)detection of small intestinal propulsive rate;(3)evaluation of contractility of colonic smooth muscle strips before and after ICC removal;(4)detection of plasma and distal colon C-kit, M-SCF, S-SCF expression by elisa method;(5)detection of colonic tissue C-kit, M-SCF protein. Results Compared with the normal group, the model group, Deshute group, and Tongxie Decoction group significantly shortened the time of glass pellet excretion(P<0.01) and increased the small intestinal propulsive rate(P<0.01). Compared with the model group, the excretory time of Deshute group and Tongxie Decoction group significantly prolonged(P<0.01) and the small intestinal propulsive rate significantly decreased(P<0.01), and there was no significant difference between Deshute group and Tongxie Decoction group(P<0.05). Compared with the normal group, the reduction rate of colon smooth muscle contraction curve in the model group was significantly increased, but it was recovered in the Deshute group and the Tongxie Decoction group. The expression level of C-kit, S-SCF, M-SCF was significantly reduced by Elisa and Western blot. Conclusion Tongxie Decoction can change the contraction of colic smooth muscle, decrease the rate of small intestine propulsion and improve the symptoms of irritable bowel syndrome by regulating C-kit/SCF signaling pathway in the treatment of IBS.
