不同磺脲类药物及胰岛素对2型糖尿病合并冠心病患者的心血管安全性分析

摘　　要：目的分析使用不同种类磺脲类药物及注射胰岛素的2型糖尿病合并冠心病患者不良心血管事件发生情况,对磺脲类药物的安全合理使用提供参考。方法选择421例本院2013年1月~2016年12月间住院的正在使用磺脲类药物或胰岛素治疗的2型糖尿病合并冠心病患者。根据入院前患者接受的不同治疗方式将其分组:(1)胰岛素组;(2)格列吡嗪组;(3)格列奇特组;(4)格列喹酮组;(5)格列美脲组。记录并分析患者的一般临床资料、相关实验室检查和特殊检查数据,并同时记录治疗效果和并发心血管终点事件。对采集数据进行统计学分析。结果不同磺脲类药物之间比较临床终点情况,经统计学分析,各组之间无统计学差异(P>0.05)。结论磺脲类药物与胰岛素相比并没有增加心血管事件的发生,不同药物之间也没有明显的差异。Objective To analyze the occurrence of adverse cardiac events induced by different kinds of sulfonylureas and injection of insulin in patients with type 2 diabetes mellitus complicated with coronary heart disease, so as to provide reference for the safety and rational use of sulfonylureas. Methods A total of 421 patients with type 2 diabetes mellitus complicated with coronary heart disease treated with sulfonylureas or insulin from January 2013 to December 2016 were selected. According to the different treatments received prior to admission to the group, patients were divided into:(1)insulin group;(2)glipizide group;(3)gliclazide group;(4)glimepiride group;(5)gliquidone group. The patient’s general clinical data, related laboratory inspection and special inspection data, treatment effect including cardiovascular endpoint events were recorded and analyzed. Results Sulfonylureas had no influence on the cardiovascular incidences and there was no significant difference between the groups(P>0.05). Conclusion Sulfonylureas do not increase the incidence of cardiovascular events compared with insulin, and there is no statistically significant difference between different drugs.
