彩色高频超声对儿童钙化上皮瘤和常见皮肤肿物的鉴别诊断分析

摘　　要：目的:分析彩色高频超声对儿童钙化上皮瘤和常见皮肤肿物的鉴别诊断价值。方法:选取医院收治的45例皮肤肿物患儿,均实施彩色高频超声检查,且均进行手术切除肿物,切除的标本均接受病理检查。统计病理学检查结果。将病理学检查结果作为"金标准",分析彩色高频超声诊断不同类型皮肤肿物的主要表现。结果:组织病理学诊断的45例患儿中钙化上皮瘤17例(占37.78%);非钙化上皮瘤28例(占62.22%),其中皮样囊肿14例,表皮样囊肿8例,脂肪瘤3例,血管瘤3例。17例钙化上皮瘤与28例非钙化上皮瘤中,彩色高频超声分别诊断出15例和26例与组织病理学结果一致。彩色高频超声对钙化上皮瘤和其他常见皮肤肿物的鉴别诊断的灵敏度、特异度、准确率以及约登指数分别为88.24%、92.86%、91.11%和0.8111。结论:彩色高频超声诊断儿童钙化上皮瘤准确度高,且可与皮样囊肿、表皮样囊肿、脂肪瘤及血管瘤相鉴别。Objective:To analyze the value of color high frequency ultrasound in differential diagnosis of calcifying epithelioma and common skin masses of children.Methods:The clinical data of 45 children with skin masses were selected.All of them received examination of color high-frequency ultrasound,and underwent surgery to excise masses,and all the excised specimens were tested by pathological examination.And the pathological results were analyzed by statistic method.The results of pathological examination were used as“Golden standard”to analyze the main manifestations of different types of skin masses which were diagnosed by color high-frequency ultrasound.Results:In 45 cases,the results of histopathologic diagnosis indicated 17 cases were calcifying epithelioma(37.78%),and 28 cases were non-calcifying epithelioma(62.22%)which included 14 cases with dermoid cysts,8 cases with epidermoid cysts,3 cases with lipomyoma and 3 cases with hemangiomas.Compared with the above results,the results of color high-frequency ultrasound indicated calcifying epitheliomas were 15 cases which were conformity with the results of histopathologic diagnosis,and non-calcifying epitheliomas were 26 cases which were conformity with the results of histopathologic diagnosis.The sensitivity,specificity,accuracy and Yoden index of color high-frequency ultrasound in differential diagnosis of calcifying epithelioma and other common skin masses were 88.24%,92.86%,91.11%and 0.8111,respectively.Conclusion:Color high-frequency ultrasound has high accuracy in diagnosing calcifying epithelioma of children,and it can identify calcifying epithelioma from dermoid cyst,epidermoid cysts,lipomyoma and hemangioma.
