摘  要:目的:观察早期肠内营养联合全程护理干预在老年重症脑卒中患者中的应用效果。方法:将2018年3月至2020年2月我院收治的90例老年重症脑卒中患者按照随机数字表法分为联合组和对照组,每组各45例。对照组进行早期肠内营养支持+常规护理干预,联合组进行早期肠内营养支持联合全程护理干预,对比2组入住ICU时间、住院时间、住院费用及并发症发生情况,并对比2组干预前后神经功能、营养指标包括血清白蛋白(ALB)、前白蛋白(PA)、总蛋白(TP)、心理状态及生活质量的变化情况。结果:和对照组比较,联合组入住ICU时间、住院时间更短,住院费用更低,并发症发生率更低(P<0.05);干预后,联合组NIHSS、SAS、SDS评分均低于干预前和对照组,而血清ALB、PA、TP水平、各项生活质量评分均高于干预前和对照组,差异均有统计学意义(P<0.05)。结论:对老年重症脑卒中患者进行早期肠内营养联合全程护理干预不仅可改善患者神经功能、营养状态、心理状态,还可减少并发症的发生,提高其生活质量,促进患者身体康复,减轻其经济负担,值得推广。Objective:To investigate the effect of early enteral nutrition combined with full-course nursing intervention in elderly patients with severe stroke.Methods:A total of 90 elderly patients with severe stroke admitted to our hospital from Mar 2018 to Feb 2020 were divided into the combined group and the control group according to the random number table,45 cases in each group.The control group received early enteral nutrition support combined eith routine nursing intervention,while the combined group received early enteral nutrition support combined with full-course nursing intervention.The ICU stay time,hospitalization time,hospitaliza⁃tion costs and postoperative complications in the two groups were compared.And changes in nerve function,nutritional indicators[serum albumin(ALB),prealbumin(PA),total protein(TP)],mental status and quality of life were compared between the two groups before and after intervention.Results:Compared with the control group,ICU stay time and hospitalization time in the combined group was significantly shorter,hospitalization costs and complications rate was significantly lower(P<0.05).After inter⁃vention,scores of NIHSS,SAS and SDS in the combined group were lower than those before intervention and in control group,while serum levels of ALB,PA,TP and the scores of quality of life were higher than those before intervention and in control group(P<0.05).Conclusion:Early enteral nutrition combined with full-course nursing intervention for elderly patients with severe stroke can not only improve the patient′s neurological function,nutritional status,and psychological status,but also reduce the occurrence of complications,improve their quality of life,promote the physical recovery,and reduce their financial burden.
