摘 要:目的观察评价分析晚断脐(DCC)与新生儿血氧含量变化的影响效果。方法选择2017年2月至2019年1月中山市陈星海医院278例正常单胎阴道分娩孕产妇按照随机数字表法随机分为两组,早断脐(ECC)组139例在新生儿娩出后立即断脐;晚断脐组139例在新生儿在脐带停止搏动后断脐,观察两组新生儿血氧含量变化。结果晚断脐组新生儿24 h红细胞、血红蛋白、细胞比容均显著高于早断脐组,差异有统计学意义(P<0.05);晚断脐组和早断脐组新生儿胆红素以及达峰时间比较,差异无统计学意义(P>0.05),晚断脐组晚发感染率为11.51%明显的低于早断脐组28.78%,两组比较差异有统计学意义(P<0.05)。结论晚断脐能有效预防新生儿贫血,使新生儿血红蛋白量升高,且未增加新生儿病理性黄疸、病理性红细胞增多症等各种疾病的发生率,未发现对新生儿有不良影响,值得临床大力推广。Objective To observe the efficacy of delayed cord clamp(DCC)in pregnant women and the change of blood oxygen content in neonates.Methods A total of 278 pregnant women with normal single birth vaginal delivery to Zhongshan Chenxinghai Hospital form February 2017 to January 2019 were selected,and they were divided into the early cord clamp(ECC)group(n=139)and the DCC group(n=139)according to the random number table method.Umbilici of the neonates in the ECC group were cut off immediately after delivery,while those of the DCC group were cut off after the umbilical cord stopped beating.Meanwhile,the changes of blood oxygen content of neonates in the two groups were observed.Results The 24-hour red blood cells,hemoglobin and hematocrit of neonates in the DCC group were significantly higher than those in the ECC group,and there was statistically significant difference between the two groups(P<0.05).There was no statistically significant difference in neonatal bilirubin and peak time between the DCC group and the ECC group(P>0.05).The delayed infection rate of the DCC group was 11.51%,which was significantly lower than 28.78%of the ECC group,and there was statistically significant difference between the two groups(P<0.05).Conclusion Delayed cord cutting can effectively prevent neonatal anemia and septicemia,increase the amount of hemoglobin in neonates without increasing the incidence of pathological jaundice,pathological polycythemia and other diseases in neonates,and improve the quality of umbilical cord.There are no adverse effects found on neonates,it is worth promoting vigorously,it is worthy of clinical promotion.
