摘 要:目的探讨基于二代测序技术的etDNA与ctDNA检测在肺癌合并胸腔积液诊断中的应用价值。方法选取2018年5—12月惠东县人民医院收治的40例晚期非小细胞肺癌合并胸腔积液患者为研究对象,收集患者的胸腔积液、血液以及原发灶肿瘤组织作为检测样本,应用二代基因测序技术检测胸腔积液DNA(et DNA)、外周血DNA(ct DNA)、原发灶肿瘤组织DNA(t DNA)的AF值及其EGFR基因突变情况。结果40例患者的etDNA的AF值较ctDNA明显升高(P<0.05),而与tDNA比较,差异无统计学意义(P>0.05)。以tDNA检测出的EGFR基因突变结果为标准,etDNA检测EGFR基因突变的敏感度和准确性明显高于ctDNA检测(P<0.05)。结论与ctDNA检测相比,应用二代测序技术检测etDNA诊断晚期非小细胞肺癌合并胸腔积液的敏感度更高,对于临床早期诊治具有良好的指导意义。Objective To investigate the application value of etDNA and ctDNA in diagnosis of pulmonary cancer complicated with pleural effusion based on the second-generation sequencing technology.Methods 40 patients with advanced non-small cell pulmonary cancer complicated with pleural effusion admitted to Huidong People’s Hospital from May 2018 to December 2018 were selected as the research objects.Pleural effusion,blood and primary tumor tissues were collected as detection samples.Meanwhile,the AF values of DNA of pleural effusion(etDNA),peripheral blood DNA(ctDNA),primary tumor tissue DNA(tDNA)and EGFR gene mutations were detected by the secondgeneration gene sequencing technology.Results The AF value of etDNA in 40 patients was significantly higher than that of ctDNA(P<0.05),but there was no statistically significant difference between etDNA and tDNA(P>0.05).According to the results of EGFR gene mutations detected by tDNA,the sensitivity and accuracy of etDNA detection of EGFR gene mutations were significantly higher than those of ctDNA detection(P<0.05).Conclusion Compared with ctDNA detection,the etDNA detection by the second-generation sequencing technology is more sensitive in the diagnosis of advanced non-small cell pulmonary cancer complicated with pleural effusion,which has a good guiding significance for early clinical diagnosis and treatment.
