单髁置换术治疗膝关节内侧间室骨性关节炎的疗效观察

摘　　要：目的观察膝关节单髁置换术(unicompartmental knee arthroplasty,UKA)治疗膝关节内侧间室骨性关节炎的临床疗效。方法选取自2013年5月至2018年11月期间收治病变累及膝关节内侧间室的膝关节骨性关节炎的患者85例,随机分为两组,观察组42例,行膝关节单髁置换术(UKA),对照组43例,采用全膝关节置换术(total knee arthroplasty TKA)治疗。通过对比膝关节疼痛视觉模拟评分表(VAS)、膝关节HSS评分、膝关节屈曲活动度(ROM)等对疗效进行评估。结果所有患者均获随访,时间1.5年至3年。术后3个月内膝关节疼痛明显减轻,膝关节功能评分及膝关节屈曲活动度均明显优于术前,差异有统计学意义(P<0.05)。结论累及内侧间室的膝关节骨性关节炎,UKA与TKA均能取得有效的临床效果,明显减轻患者的痛苦,改善膝关节的功能,但UKA创伤小,术后恢复快,在严格把握适应症的前提下,值得在临床中应用。Objective To observe the curative effect of unicompartmental knee arthroplasty(UKA)on single compartment osteoarthritis of knee.Methods 85 cases with single compartment osteoarthritis of knee hospitalized during the period from May,2013 to Nov.,2018 were selected and randomly divided into 2 groups:observation group(n=42,undergoing UKA)and control group(n=43,undergoing total knee arthroplasty,TKA);visual analogue score(VAS),hospital for special surgery(HSS)score and range of motion(ROM)were applied in evaluating the curative effect.Results Followup lasted from 1.5 to 3 years;in the first 3 months after surgery,the knee joint pain of the cases in both groups were obviously relieved,the HSS scores and Rom were much superior to those before surgery with statistical difference(P<0.05).Conclusions Both UKA and TKA can produce clinical curative effect on single compartment osteoarthritis of knee,obviously relieve the pain of the cases and improve the knee joint function while UKA has the advantage of small wound and fast heal after surgery,which is worthy of wide clinical application under strict control of indications.
