TSH、血脂水平及肾功能、甲状腺功能与2型糖尿病患者肾损伤的相关性

摘　　要：目的探讨促甲状腺激素(TSH)、血脂水平及肾功能在2型糖尿病肾损伤患者血清中的表达及与甲状腺功能的相关性。方法89例2型糖尿病肾损伤患者作为合并组,同期治疗的2型糖尿病患者71例作为糖尿病组;采用全自动生化分析仪测定患者血清甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)及低密度脂蛋白胆固醇(LDL-C)水平;采用肾功能测定仪完成患者尿素氮(BUN)、血清肌酐(CRE)和尿酸(UA)水平;采用化学发光法测定各组游离三碘甲状腺原氨酸(FT3)、游离甲状腺激素(FT4)、TSH水平;采用Pearson相关性分析2型糖尿病患者TSH、血脂及肾功能水平与甲状腺功的相关性。结果合并组TC、TG、LDL-C、BUN、CRE、UA及TSH水平高于糖尿病组,HDL-C、FT3水平低于糖尿病组,差异有统计学意义(P<0.05);Pearson相关性分析结果显示,2型糖尿病肾损伤患者TC、TG、LDL-C、BUN、CRE及UA与FT3水平呈负相关性(P<0.05),与HDL-C呈正相关性(P<0.05),2型糖尿病肾损伤患者TC、TG、LDL-C、BUN、CRE及UA与TSH呈正相关性(P<0.05),与HDL-C呈负相关性(P<0.05)。结论2型糖尿病患者常伴有血脂、肾功能异常,且与甲状腺功能存在相关性。Objective To explore the expression of thyroid stimulating hormone(TSH),blood lipid levels,and renal function of type 2 diabetic patients with kidney injury and its correlation with thyroid function.Methods Eighty-nine patients with type 2 diabetic kidney injury were selected as the combined group,and 71 patients with type 2 diabetes treated in the same period were selected as the diabetic group.The level of triglyceride(TG),total cholesterol(TC),high-density lipoprotein cholesterol(HDL-C),and low-density lipoprotein cholesterol(LDL-C)of the patients were measured by the automatic biochemical analyzer.The levels of urea nitrogen(BUN),serum creatinine(CRE),and uric acid(UA)were determined by the renal function meter and the levels of free triiodothyronine(FT3),free thyroid hormone(FT4),and thyroid stimulating hormone(TSH)were detected with the chemiluminescence method.Then Pearson correlation analysis was used to analyze the correlation between TSH,blood lipid,and renal function levels and thyroid function in patients with type 2 diabetes.Results Compared with the diabetic group,the combined group had higher levels of TC,TG and LDL-C(P<0.05),lower levels of HDL-C(P<0.05),higher levels of BUN,CRE,UA,and TSH(P<0.05),and lower FT3 level(P<0.05).Pearson correlation analysis results showed that the levels of TC,TG,LDL-C,BUN,CRE,and UA of patients with type 2 diabetic kidney injury were negatively correlated with their FT3 levels(P<0.05),and positively correlated with HDL-C(P<0.05),and positively correlated with TSH(P<0.05).while with thier HDL-C was negatively correlated(P<0.05).Conclusion Patients with type 2 diabetes are often accompanied by abnormal blood lipids and renal function,and there is a correlation with thyroid function.
