摘  要:目的:探究生物反馈电刺激对产后盆底脏器脱垂患者盆底结构参数及盆底肌力的影响.方法:选取2018年6月-2020年2月期间重庆市妇幼保健院86例产后盆底脏器脱垂患者为研究对象,将其随机分为对照组(盆底肌肉锻炼组)和观察组(盆底肌肉锻炼联合生物反馈电刺激治疗组),每组各43例.比较两组治疗前后的盆腔器官脱垂定量(POP-Q,pelvic organ prolapse quantitation)分级、盆底结构参数及盆底肌力.结果:治疗前两组的POP-Q分级、盆底结构参数及盆底肌力比较,差异无统计学意义(p>0.05).治疗后4周及8周两组上述方面均持续改善,观察组POP-Q 0级、Ⅰ~Ⅱ级及Ⅲ~Ⅳ级者占比分别为19例(44.19%)、21例(48.84%)、3例(6.98%)及29例(67.44%)、13例(30.23%)、1例(2.33%);盆底结构参数尿道旋转角(UR,urethral rotation)、膀胱颈距耻骨联合的移动度(BND,bladder neck descent)及尿道倾斜角(UI,urethral inclination)分别为(47.51±3.79)°,(18.26±1.78)mm,(5.59±0.91)°及(40.23±3.63)°,(13.63±1.63)mm,(1.98±0.33)°;盆底Ⅰ类及Ⅱ类纤维肌力异常率分别为48.84%,18.61%及39.53%,13.95%,上述方面均显著优于对照组,差异具有统计学意义(p<0.05).结论:盆底肌肉锻炼联合生物反馈电刺激治疗可显著改善产后盆底脏器脱垂患者的盆底结构参数及盆底肌力,在此类患者中的应用价值较高.Objective:To investigate the influence of"pelvic floor muscle exercise combined with biofeedback-electrical stimulation therapy"on postpartum pelvic floor structural parameters and pelvic floor muscle strength of patients with postpartum pelvic floor organ prolapse.Methods:Eighty-six patients with postpartum pelvic floor organ prolapse from June 2018 to February 2020 were chosen as the research object,and they were randomly divided into acontrol group(pelvic floor muscle exercise group)and observation group(pelvic floor muscle exercise combined with biofeedback-electrical stimulation therapy group)with 43 cases in each.Then the pelvic organ prolapse quantitation(POP-Q)classifications,pelvic floor structural parameters and pelvic floor muscle strength of thetwo groups before and after the treatment were compared.Results:The POP-Q classifications,pelvic floor structural parameters and pelvic floor muscle strength of the two groups before the treatment were compared,there were no statistically significant differences(p>0.05).Four and eight weeks after the treatment,these items steadily improved in both groups,and the POP-Q classifications grade 0,gradeⅠtoⅡand gradeⅢtoⅣwere respectively 19 cases(44.19%),21 cases(48.84%)and 3 cases(6.98%),and 29 cases(67.44%),13 cases(30.23%)and 1 case(2.33%);the pelvic floor structural parameters urethral rotation(UR),bladder neck descent(BND)and urethral inclination(UI)were respectively(47.51±3.79)°,(18.26±1.78)mm,(5.59±0.91)°and(40.23±3.63)°,(13.63±1.63)mm,(1.98±0.33)°,andthe abnormal rates of pelvic floor type Ⅰ and type Ⅱ fibromuscular strength were respectively 48.84%,18.61%and 39.53%,13.95%,and they were all significantly(p<0.05)better than those of the control group.Conclusion:The pelvic floor muscle exercise combined with biofeedback-electrical stimulation therapy can significantly improve the postpartum pelvic floor structural parameters and pelvic floor muscle strength of patients with postpartum pelvic floor organ prolapse,so it is of considerableappl
