摘 要:目的通过256排螺旋CT血管造影观察肝固有动脉的解剖结构,探讨肝固有动脉的分支分型及变异率,为肝脏疾病的诊治提供参考依据。方法通过收集2017年6月至2019年6月间在湖南中医药大学附属第一医院影像科行上腹部CT增强扫描的患者80例,统计和分析肝固有动脉的分支分型情况,测量其相关数据。结果本组肝固有动脉主干长度为(45.44±22.76)mm,起点处内径为(2.66±1.68)mm;肝左动脉主干长度为(43.68±18.75)mm,起点处内径为(3.23±1.09)mm,肝内分支分布的4种类型分别占72.5%、2.5%、1.25%、23.75%;肝中动脉的出现率为25%,其主干长度为(59.21±23.75)mm,起点处内径为(1.99±0.71)mm;肝右动脉主干长度为(43.10±18.50)mm,起点处内径为(3.26±1.08)mm,肝内分支分布的5种类型分别占71.25%、20%、3.75%、3.75%、1.25%。结论肝固有动脉及其分支解剖结构复杂多样,充分了解其正常走行规律及变异情况,可为临床上腹部外科手术的开展提供指导。Objective To observe the anatomical structure of the proper hepatic artery by 256-slice spiral CT angiography,and to discuss the branch and typing and variation rate of the proper hepatic artery,so as to provide reference forthe diagnosis and treatment of liver diseases.Methods 80 patients who received enhanced UPPER abdominal CT scan inthe department of Imaging of the First Affiliated Hospital of Hunan University of Chinese Medicine from June 2017 to June2019 were collected,and the branch typing of the proper hepatic artery was counted and analyzed,and the relevant datawere measured.ResultsIn this group,the main trunk length of the proper hepatic artery was(45.44±22.76)mm,and theinternal diameter at the starting point was(2.66 ± 1.68)mm. The length of the main trunk of the left hepatic artery was(43.68±18.75)mm,and the internal diameter at the starting point was(3.23±1.09)mm. The distribution of the four types ofbranches in the liver accounted for 72.5%,2.5%,1.25% and 23.75% respectively. The occurrence rate of the middle hepat-ic artery was 25%,the trunk length was(59.21±23.75)mm,and the starting diameter was(1.99±0.71)mm. The length ofthe main trunk of the right hepatic artery was(43.10±18.50)mm,the internal diameter at the starting point was(3.26±1.08)mm,and the distribution of the 5 types of branches in the liver accounted for 71.25%,20%,3.75%,3.75%,and 1.25%,respectively.Conclusion The anatomical structure of the proper hepatic artery and its branches is complex and diverse,and to fully understand its normal operation and variation can provide guidance for clinical abdominal surgery.
