三种问卷量表对妊娠期阻塞性睡眠呼吸暂停低通气综合征筛查价值的比较

摘　　要：目的评价Epworth嗜睡量表、STOP-Bang问卷和Berlin问卷对妊娠期OSASH的筛查价值。方法选取2019年6月至2020年6月于江西省妇幼保健院住院的打鼾孕妇87例作为研究对象,对受试者进行ESS、SBQ和Berlin问卷评分,并采用多导睡眠仪对受检者进行整夜不少于7h的PSG监测,采用ROC曲线比较三种量表对OSAHS的诊断效能。结果分别取AHI为5、15、30次/h作为截点时,ESS的曲线下面积分别0.780、0.797、0.842;SBQ的曲线下面积为0.787、0.844、0.807;Berlin的曲线下面积为0.844、0.793、0.822。而三者联合筛查时曲线下面积达0.911。结论ESS、SBQ和Berlin对妊娠期OSAHS均有一定的筛查价值,但三者联合使用时可获得较高的筛查效能。Objective To evaluate the screening value of Epworth drowsiness scale,STOP-Bang questionnaire and Berlin questionnaire for gestational OSASH.Methods 87 cases of snoring pregnant women from Jiangxi Maternal and Child Health Hospital were selected as subjects from June 2019 to June 2020.Their clinical data were analyzed,including ESS score,STOP-Bang questionnaire and Berlin questionnaire and underwent overnight polysomnography(PSG)were enrolled in this study.The sensitivity,specificity,positive predictive value and negative predictive value of each questionnaire was statistically analyzed by receiver operating characteristic(ROC)curve,and the efficacy of three questionnaires were compared.Results When AHI were taken 5、15、30 times/h as intercept points,the AUC of ESS score were 0.780、0.797、0.842;the AUC of SBQ were 0.787、0.844、0.807;the AUC of Berlin questionnaire were 0.844、0.793、0.822.When combined,the area under the curve was 0.911.Conclusion ESS、SBQ and Berlin have certain screening value for pregnancy OSAHS,and can obtain higher screening efficiency when they are used together.
