藏、汉族OSAHS患者血清低氧相关因子水平与肺动脉收缩压的相关性研究

摘　　要：目的探讨藏、汉族阻塞性睡眠呼吸暂停低通气综合征(OSAHS)患者血清低氧相关因子水平与其临床指标的相关性。方法回顾性研究。入选2017年1月至2019年12月在青海大学附属医院呼吸与危重症医学科确诊的藏、汉族OSAHS患者各30例为病例组,此外,以在青海大学附属医院体检正常的藏、汉族志愿者各30名为健康对照组,所有受试者平均居住海拔为2260 m。比较2组受试者的基线资料;测定血清中低氧诱导因子1α(HIF-1α)、血管内皮生长因子(VEGF)、血管内皮生长因子受体(VEGFR)、促红细胞生成素、促红细胞生成素受体浓度;比较2组受试者的临床指标如呼吸暂停低通气指数(AHI)、肺动脉收缩压(PASP)、红细胞计数(RBC)、血红蛋白(Hb)和红细胞压积(HCT);对HIF-1α、VEGF和VEGFR水平与PASP进行相关性分析。结果2组受试者基线资料以及血清学指标差异均无统计学意义(P值均>0.05);与健康对照组比较,藏、汉族OSAHS组患者AHI、PASP、RBC、Hb和HCT显著升高(P值均<0.05);汉族OSAHS患者血清HIF-1α、VEGF、VEGFR水平与PASP呈正相关(r=0.217、0.286、0.153,P值均<0.05);藏族OSAHS患者血清HIF-1α、VEGF、VEGFR水平与PASP呈正相关(r=0.242、0.322、0.020,P值均<0.05)。结论在间歇低氧状态下,藏、汉族OSAHS患者PASP升高。另外,AHI、Hb、RBC和HCT显著升高,提示OSAHS患者可发生继发性红细胞增多。藏、汉族OSAHS患者血清HIF-1α、VEGF和VEGFR水平均与PASP呈正相关。Objective To investigate the correlation between serum levels of hypoxia related factors and their clinical parameters in Zang and Han nationalities patients with obstructive sleep apnea hypopnea syndrome(OSAHS).Methods 30 Zang and Han nationalities OSAHS patients each diagnosed at Department of Respiratory and Critical Care Medicine,Qinghai University Hospital between January 2017 and December 2019 were enrolled as the case group,and in addition,30 Zang and Han nationalities volunteers each diagnosed at Qinghai University Hospital with normal physical examination were enrolled as the control group,and all subjects lived at an average altitude of 2260 m.Baseline data were compared between the two groups of subjects.Serum concentrations of hypoxia-inducible factor-1α(HIF-1α),vascular endothelial growth factor(VEGF),vascular endothelial growth factor receptor(VEGFR),erythropoietin,erythropoietin receptor were measured by enzyme linked immunosorbent assay.The clinical parameters that compared the two groups of subjects included apnea-hypopnea index(AHI),pulmonary artery systolic pressure(PASP),red blood cell count(RBC),hemoglobin(Hb),and hematocrit(HCT).Correlations between HIF-1α,VEGF,VEGFR levels and PASP were performed.Results The baseline data as well as serological parameters of the subjects in both groups were not statistically significant(all P>0.05).AHI,PASP,RBC,Hb and HCT were significantly higher in Zang nationality OSAHS patients compared with healthy controls(all P<0.05).The serum levels of HIF-1α,VEGF and VEGFR in Han nationalities patients with OSAHS were positively correlated with PASP(r=0.217,0.286,0.153,all P<0.05).The serum levels of HIF-1α,VEGF and VEGFR were positively correlated with PASP in Zang nationality patients with OSAHS(r=0.242,0.322,0.020,all P<0.05).Conclusions PASP is elevated in OSAHS of Zang and Han Chinese nationalities patients under intermittent hypoxic conditions.In addition,AHI,Hb,RBC and HCT were significantly higher,suggesting that secondary erythrocytosis can occur in p
