摘 要:目的:探讨肿瘤科患者留置经外周静脉置入中心静脉导管(PICC)后,透明敷料不同更换频率对导管相关感染发生的影响,为选择合适的透明敷料更换频率提供可靠依据.方法:选择2018年11月至2019年1月在本院肿瘤科留置PICC导管的120例患者,其中男76例,女44例,随机分为4组:透明敷料7、8、9、10 d组,每组患者30例,比较4组患者PICC导管相关感染发生情况.结果:120例肿瘤患者中有3例发生过敏性皮炎,其中9 d组1例、10 d组2例;16例发生导管口感染,7 d组、8 d组、9 d组、10 d组的导管口感染率分别为16.67%、13.33%、13.33%、10.00%,差异无统计学意义(χ2=0.577,P>0.05),所有患者均未发生导管相关性血流感染.结论:PICC透明敷料更换频率可适当延长,但不超过10 d更换一次.Objective:To investigate the effect of different replacement frequency of transparent dressing on the occurrence of catheter-related infection after indwelling peripherally inserted central catheter(PICC) in patients of oncology department, and provide a reliable basis for selecting suitable transparent dressing replacement frequency. Methods: A total of 120 patients with PICC in oncology department of our hospital from November 2018 to January 2019, including 76 males and 44 females, were selected and randomly divided into 4 groups according to the random number table: 7-day replacement group, 8-day replacement group, 9-day replacement group, 10-day replacement group, 30 patients in each group. And analyze and compare the occurrence of PICC catheter-related infections in 4 groups of patients. Results: Allergic dermatitis occurred in 3 of 120 tumor patients, including 1 in the 9 d group and 2 in the 10 d group. Catheter infection occurred in 16 of 4 groups patients, the infection rates of catheterization in the 7 d, 8 d, 9 d, and 10 d groups were 16.67%, 13.33%, 13.33%, and 10.00%, respectively, and the difference was not statistically significant( χ2=0.577, P>0.05). No catheter-related bloodstream infection occurred in all patients. Conclusion:The replacement frequency of PICC transparent dressing can be extended appropriately, but not more than once every 10 days.
