摘  要:目的观察清金贝蒌汤联合无创正压通气治疗痰热壅肺型支气管扩张症的临床价值。方法选取2019年1月—2020年9月收治的120例支气管扩张症患者,按照随机数字表法分为两组,每组60例。常规对症支持治疗基础上,对照组给予无创正压通气,治疗组给予清金贝蒌汤+无创正压通气治疗,两组均治疗7 d。比较两组临床疗效、治疗安全性、治疗前后病情评分(BSI评分、APACHEⅡ评分)、肺功能[第1秒用力呼气容积(FEV_(1))、用力肺活量(FVC)及1 s率(FEV_(1)/FVC)]改善情况及血清炎症因子[α防御素1-3(HNP1-3)、单核细胞趋化蛋白1(MCP-1)、基质金属蛋白酶-9(MMP-9)]水平变化。结果治疗组治疗后总有效率高于对照组(P<0.05);两组治疗后BSI、APACHEⅡ评分均较本组治疗前降低,且治疗组低于对照组(P<0.05);两组治疗后FEV_(1)、FVC、FEV_(1)/FVC水平均较本组治疗前增加,且治疗组高于对照组(P<0.05);治疗后两组血清HNP1-3、MCP-1、MMP-9水平均较本组治疗前降低,且治疗组低于对照组(P<0.05),差异有统计学意义。结论清金贝蒌汤联合无创正压通气治疗支气管扩张症(痰热壅肺型)安全、有效,可有效缓解病情,改善患者肺功能,可能与下调血清炎症介质表达,减轻支气管炎症反应有关。Objective To explore the clinical value of Qingjin Beilou Decoction combined with non-invasive positive pressure ventilation in the treatment of phlegm-heat obstruction bronchiectasis.Method A total of 120 patients with bronchiectasis admitted from January 2019 to September 2020 were selected and divided into two groups according to the random number table method,with 60 cases in each group.On the basis of conventional symptomatic and supportive treatment,the control group was given non-invasive positive pressure ventilation,and the treatment group was given Qingjin Beilou Decoction+non-invasive positive pressure ventilation.Both groups were treated for 7 days.The clinical efficacy,safety of treatment,disease scores before and after treatment(BSI score,APACHEⅡscore),lung function[forced expiratory volume in the first second(FEV_(1)),forced vital capacity(FVC)and 1-second rate(FEV_(1)/FVC)were compared between the two groups]Improve the situation and serum inflammatory factors[αdefensin 1-3(HNP1-3),monocyte chemotactic protein 1(MCP-1),matrix metalloproteinase-9(MMP-9)]level changes.Result The total effective rate of treatment in the treatment group was higher than that in the control group(P<0.05);the scores of BSI and APACHEⅡafter treatment in the two groups were lower than those before treatment in this group,and the treatment group was lower than the control group(P<0.05);The levels of FEV_(1),FVC,FEV_(1)/FVC were higher than those of this group before treatment,and the treatment group was higher than that of the control group(P<0.05);after treatment,the serum levels ofHNP1-3,MCP-1,and MMP-9 in the two groups were higher than those in this group Decrease before treatment,and the treatment group is lower than the control group(P<0.05),the difference is statistically significant.Conclusion Qingjin Beilou Decoction combined with non-invasive positive pressure ventilation is safe and effective in the treatment of bronchiectasis(phlegm-heat entrapment of the lung).It can effectively alleviate the disease and i
