摘  要:目的:探究还原型烟酰胺腺嘌呤二核苷酸磷酸(NADPH)氧化酶4(NOX4)、炎症细胞因子IL-6、hs-CRP水平与房颤的关联性及其对房颤的诊断价值,并分析房颤发病的危险因素。方法:选取某院心内科诊断的非瓣膜性心房颤动患者80例作为病例组,其中阵发性房颤患者40例(PaAF组),持续性房颤患者40例(PeAF组),另选取同期医院健康体检者(窦性心律)38例作为对照组。检测研究对象血清NOX4、IL-6、hs-CRP水平,评估血清NOX4、IL-6、hs-CRP水平与房颤的关联性,采用二元Logistic回归分析影响房颤发病的危险因素,ROC曲线评估血清NOX4、IL-6、hs-CRP水平诊断房颤发病的效能。结果:血清NOX4、IL-6、hs-CRP水平在对照组、阵发性房颤组、持续性房颤组依次升高,组间比较差异有统计学意义(P<0.05)。二元Logistic回归分析显示NOX4、IL-6、hs-CRP是房颤发病的影响因素(P<0.05)。研究对象外周血NOX4、IL-6、hs-CRP表达水平与LAD呈正相关性(r=0.376、0.638、0.258,P<0.05)。血清NOX4、IL-6、hs-CRP值诊断房颤发病的ROC曲线下面积分别为0.774、0.894、0.700。结论:NOX4、IL-6、hs-CRP表达水平与房颤的持续时间有关,是房颤发病的影响因素,对房颤发生具有一定预测价值。Objective:To investigate the relationship between reduced nicotinamide adenine dinucleotide phosphate(NADPH)oxidase 4(NOX4),inflammatory cytokine interleukin 6(IL-6),hs-CRP levels and atrial fibrillation,as well as their diagnostic value on atrial fibrillation,and to analyze the risk factors of atrial fibrillation.Methods:80 patients with non-valvular atrial fibrillation diagnosed in a hospital were selected as the research objects,including 40 patients with paroxysmal atrial fibrillation and 40 patients with persistent atrial fibrillation.Another 38 cases of healthy people(sinus rhythm)were randomly selected as the control group.Levels of NOX4,IL-6,and hs-CRP of the subjects were detected and the relationship between NOX4,IL-6,hs-CRP and atrial fibrillation was evaluated.Binary logistic regression was used to analyze the influencing factors of atrial fibrillation,and receiver operating characteristic(ROC)curve analysis was employed to assess the value of serum NOX4,IL-6 and hs-CRP levels in the onset of atrial fibrillation.Results:Levels of serum NOX4,IL-6,and hs-CRP increased in proper order in the control group,the paroxysmal atrial fibrillation group and the persistent atrial fibrillation group and the difference between the groups were statistically significant.Binary logistic regression analysis indicated that NOX4,IL-6 and hs-CRP were the risk factors of atrial fibrillation(P<0.05).The expression levels of NOX4,IL-6,hs-CRP in peripheral blood of patients with atrial fibrillation were positively correlated with LAD(r=0.376,0.638,0.258,P<0.05).ROC curve suggested that serum NOX4,IL-6 and hs-CRP values had predictive value for the onset of atrial fibrillation(the areas under the curve were 0.774,0.894,0.700,respectively).Conclusion:The expression levels of NOX4,IL-6,and hs-CRP are related to the duration of atrial fibrillation,are risk factors for the onset of atrial fibrillation,and have certain predictive value for the occurrence of atrial fibrillation.
