摘 要:目的探讨先天性心脏病(GHD)患儿在体外循环(CPB)下治疗后经规范化早期胃肠内营养对糖代谢水平及肠胃功能的影响。方法选取我院收治的100例CPB下CHD矫治治疗后患儿作为研究对象,按随机数字表法分为观察组和对照组,每组50例。对照组给予常规营养支持;观察组给予规范化早期胃肠营养方案,对比两组患儿治疗前后胃泌素、胃动素、糖代谢水平。结果观察组治疗后24 h、48 h血糖较对照组低(P<0.05),观察组治疗后72 h血糖水平与对照组比较,差异无统计学意义(P>0.05);观察组治疗后24 h、48 h胃泌素、胃动素水平较对照组高(P<0.05),观察组治疗后72 h胃泌素、胃动素水平与对照组比较,差异无统计学意义(P>0.05)。结论早期肠内营养可调整治疗后糖代谢,维持血糖应激程度,促进胃肠功能正常水平,平衡胃泌素和胃动素的分泌,提高胃泌素和胃动素水平,减少胃肠道并发症的发生。Objective To explore the effect of standardized early gastrointestinal nutrition on glucose metabolism and gastrointestinal function of congenital heart disease(GHD)children who were treated under cardiopulmonary bypass(CPB).Methods 100 children with CHD treated under CPB were selected and randomly divided into the control group and the observatiob group,50 cases in each group.The control group were given conventional nutritional support and the observation group were given standardized early gastrointestinal nutrition program.The levels of gastrin,motilin and glu⁃cose metabolism of the two groups before and after treatment were compared.Results The blood glucose of the observa⁃tion group at 24 h and 48 h after treatment was lower than those of the control group(P<0.05),and there was no significant difference in the level of glucose at 72 h after treatment between the observation group and the control group(P>0.05).The levels of gastrin and motilin in the observation group at 24 h and 48 h after treatment were higher than those in the control group(P<0.05),and there were no significant difference in he levels of gastrin and motilin at 72 h after treatment between the observation group and the control group(P>0.05).Conclusion Early enteral nutrition can adjust glucose me⁃tabolism,maintain blood glucose stress,promote the normal level of gastrointestinal function,balance the secretion of gas⁃trin and motilin,increase the serum concentration of gastrin and motilin and reduce gastrointestinal complications.
