摘　　要：目的 探讨超声刀睾丸切除术新术式的疗效及安全性。方法 回顾性分析2017年6月至2019年6月于武汉大学中南医院确诊为晚期前列腺癌并行睾丸切除术的69例患者作为研究对象,按照手术方式分为传统手术组和超声刀手术组,传统手术组27例行传统睾丸切除术,超声刀手术组42例行超声刀睾丸切除术,比较两组手术时间、术中出血量、术后住院时间、术后并发症的发生率。结果 超声刀手术组的手术时间和术中出血量均少于传统手术组,差异有统计学意义(P<0.05);两组患者的术后住院时间比较差异无统计学意义(P>0.05);传统手术组的术后并发症发生率为18.52%,而超声刀手术组术后无并发症发生,差异有统计学意义(P<0.05)。结论 超声刀睾丸切除术手术时间短、术中出血少、操作简单、并发症少,有可能成为替代传统睾丸切除术的新术式。Objective To investigate the efficacy and safety of a new method of orchiectomy with ultrasonic scalpel.Methods A retrospective analysis was performed on 69 patients with advanced prostate cancer undergoing orchiectomy in our hospital during Jun.2017 and Jun.2019.According to the surgical methods,the subjects were divided into traditional surgery group(n=27)and ultrasonic scalpel surgery group(n=42).The operation time,intraoperative blood loss,postoperative hospital stay and postoperative complication rate were compared.Results The ultrasonic scalpel surgery group had significantly shorter operation time and less intraoperative blood loss than the traditional surgery group(P<0.05).There was no significant difference in postoperative hospital stay between the two groups(P>0.05).The incidence of postoperative complications was 18.52%and 0%in the traditional surgery group and ultrasonic scalpel surgery group,respectively(P<0.05).Conclusion Ultrasonic scalpel orchiectomy has advantages of short operation time,little intraoperative bleeding,simple operation and few complications.It may be a new operation to replace the traditional orchiectomy.
