摘　　要：目的研究经尿道前列腺等离子双极电切术(TPKRP)治疗良性前列腺增生(BPH)的临床价值。方法选取2017年5月至2018年5月河北省石家庄市第一医院收治的200例BPH患者作为研究对象。试验组(n=100)行TPKRP,对照组(n=100)行电切术。比较两组手术相关指标、视觉模拟评分法(VAS)评分以及并发症发生率。比较两组手术前后国际前列腺症状评分法(IPSS)评分、国际勃起功能指数(IIEF-5)评分、早泄患者性功能-5(CIPE-5)评分、生活质量综合评定问卷(GQOLI-74)评分以及血清前列腺特异性抗原(PSA)、前列环素(PGI2)水平。结果试验组手术相关指标、VAS评分、并发症发生率均显著优于对照组,差异具有统计学意义(P<0.05)。术后,试验组IPSS评分显著低于对照组,试验组IIEF-5、CIPE-5、GQOLI-7评分以及PSA、PGI2水平均显著优于对照组,差异具有统计学意义(P<0.05)。结论TPKRP治疗BPH,能有效缓解症状,提高血清PSA、PGI2水平,改善术后性功能、生活质量。Objective To investigate the clinical value of transurethral plasmakinetic resection of prostate(TPKRP)in the treatment of benign prostatic hyperplasia(BPH).Methods 200 patients with BPH admitted to the First Hospital of Shijiazhuang City,Hebei Province from May 2017 to May 2018 were selected.The experimental group(n=100)was treat with TPKRP,while the control group(n=100)underwent electrogasification.Surgical indicators,visual analogue scale(VAS)scores and complication rates were compared between the two groups.The scores of IPSS,IIEF-5,CIPE-5,GQOLI-74,and serum PSA and PGI2 levels were compared between the two groups before and after surgery.Results The operation related indexes,VAS score and complication rate in the experimental group were significantly better than those in the control group,with statistically significant differences(P<0.05).After surgery,IPSS score of the experimental group was significantly lower than that of the control group,and IIEF-5,CIPE-5,GQOLI-7,PSA and PGI2 levels of the experimental group were significantly better than those of the control group,with statistically significant differences(P<0.05).Conclusions TPKRP in the treatment of BPH can effectively alleviate symptoms,improve levels of serum PSA and PGI2,improve postoperative sexual function and life quality.
