摘　　要：目的探讨有阴道分娩史与剖宫产史的已育妇女在人工流产术中同时放置吉妮致美宫内节育器(IUD)的临床效果。方法选择2017年11月至2018年5月在江苏省淮安市妇幼保健院行人工流产的100例已育妇女作为研究对象。根据不同分娩方式史分为A组(n=50,剖宫产史)和B组(n=50,阴道分娩史)。两组均在人工流产术中同时放置吉妮致美IUD。比较两组术前孕囊、血红蛋白(Hb)、彩超监测IUD距宫底距离、术中测量宫腔深度情况,临床随访6个月,观察两组手术前后Hb、行经天数变化,并记录月经量变化、阴道不规则出血、白带增多、IUD脱落及带器妊娠发生率等情况。结果A组术前孕囊、Hb、彩超监测IUD距宫底距离与B组比较,差异无统计学意义(P>0.05);A组术中测量宫腔深度大于B组,差异具有统计学意义(P<0.05)。两组术后6个月Hb、行经天数与术前比较,差异无统计学意义(P>0.05);A组术后Hb、行经天数与B组比较,差异无统计学意义(P>0.05)。A组月经量变化、不规则出血、白带增多、IUD脱落、带器妊娠发生率与B组比较,差异无统计学意义(P>0.05)。结论阴道分娩与剖宫产后已育妇女在人工流产术中同时放置吉妮致美IUD的操作简便、安全,且不良反应小,避孕效果好。Objective To investigate the clinical effect of placing GeneFiex IUD during induced abortion in pregnant women with a history of vaginal delivery or cesarean section.Methods A total of 100 pregnant women who underwent induced abortion in Huai′an Maternal and Child Health Hospital from November 2017 to May 2018 were selected as the research subjects.According to the history of different delivery modes,they were divided into group A(n=50,cesarean section history)and group B(n=50,vaginal delivery history).Both groups were placed with GeneFiex IUD during induced abortion.The preoperative gestational sac,Hb,IUD distance from the uterus floor monitored by color Doppler,and the intrauterine depth was measured.The clinical follow-up was 6 months,to observe the changes of Hb and the number of days before and after surgery,and record the changes of menstrual volume,irregular vaginal bleeding,increased leucorrhea,IUD shedding and pregnancy rates with IUD.Results Compared with group B,there was no statistically significant difference in the preoperative pregnancy sac,Hb and IUD distance from the uterus floor monitored by color Doppler in group A(P>0.05).The intrauterine depth in group A was greater than that in group B(P<0.05).There were no statistically significant differences in Hb and menstrual days 6 months after operation between the two groups compared with that before operation(P>0.05).There were no statistically significant differences in postoperative Hb and menstrual days between group A and group B(P>0.05).There was no significant difference in the incidence of menstruation,irregular bleeding,increased leucorrhea,IUD shedding and pregnancy rates with IUD between group A and group B(P>0.05).Conclusions It is easy and safe to place GeneFiex IUD during induced abortion in pregnant women after vaginal delivery or cesarean section,with small adverse reactions and high-efficiency contraception.
