知觉压力对不育患者精子染色质结构完整性的影响因素分析

摘　　要：目的探讨男性不育患者心理状态(知觉压力、焦虑、抑郁)及不良生活习惯等对其精子染色质结构完整性损伤的影响,为临床有效护理干预及健康指导提供可靠依据。方法选取2019年1月至5月兰州大学第一医院生殖医学中心诊治的208例男性不育患者进行调查。采用中文版压力知觉量表(CPSS)、综合性医院焦虑抑郁量表(HAD)及一般情况调查表进行调查,并对结果进行统计分析。结果患者CPSS得分为(23.27±7.10)分(5~40分)。HAD焦虑维度得分为(5.94±3.12)分(0~16分),抑郁维度得分为(6.17±2.97)分(0~16分)。多元线性回归结果显示,知觉压力、年龄为男性不育患者精子DNA碎片指数(DFI)的独立影响因素(P<0.05),且呈正相关;每日久坐时长为精子高DNA染色性的独立影响因素(P<0.05)。结论男性不育患者知觉压力、年龄、每日久坐时长是其精子染色质结构完整性的重要影响因素,因此要关注男性不育患者心理健康及其对精液质量的影响,尤其是对精子染色质结构完整性差的患者,需制定更有效的心理护理及专科护理干预措施。Objective To explore the effects of psychological state(perceived stress,anxiety,depression)and bad living habits on the integrity of sperm chromatin structure in male infertile patients,so as to provide reliable basis for clinical effective nursing interventions and health guidance.Methods 208 male infertile patients treated in Reproduction Medicine Center of the First Hospital of Lanzhou University from January to May 2019 were selected as the research subjects.Chinese version of perceived stress scale(CPSS),general hospital anxiety and depression scale(HAD)and general condition questionnaire were used for investigation,and the results were statistically analyzed.Results The CPSS scores of patients were 23.27±7.10(5~40).The score of anxiety dimension of HAD was 5.94±3.12(0~16),and that of depression was 6.17±2.97(0~16).The results of multiple linear regression showed that age and perceived stress are independent influencing factors of DNA fragmentation index in male infertile patients(P<0.05),and they were positively correlated;besides the sedentary time of each day was an independent factor of high DNA stainability(P<0.05).Conclusions The perceived stress,age and daily sedentary time of male infertile patients are the important factors affecting the integrity of sperm chromatin structure.Therefore,it is necessary to pay attention to the mental health of male infertile patients and its influence on semen quality,especially for the patients with poor sperm chromatin structural integrity,and to develop more effective psychological nursing and specialized nursing intervention measures.
