摘　　要：目的探讨美沙酮维持治疗(MMT)对女性海洛因依赖患者性功能及谷丙转氨酶(GPT)、脂蛋白a[Lp(a)]水平的影响。方法选取2018年11月至2019年11月在武汉市民主路社区药物维持治疗专科门诊部、劳动社区健康门诊部、徐家棚社区药物维持治疗专科门诊部、卫民健康门诊部和武东医院精神科门诊部就诊的100例女性海洛因依赖患者作为研究对象。口服美沙酮进行维持治疗1年。采用女性性功能指数(FSFI)问卷对患者性功能进行评估,比较治疗前后GPT、尿素氮(BUN)水平,及Lp(a)、血脂水平,观察患者治疗前后月经情况,并评估患者生存质量。结果MMT后患者性欲望、性唤起和性高潮评分低于治疗前,阴道润滑和性交痛、性生活满意状况、总体性功能评分高于治疗前,差异具有统计学意义(P<0.05)。MMT后患者GPT水平与治疗前相比,差异无统计学意义(P>0.05);治疗后BUN水平高于治疗前,Lp(a)水平低于治疗前,高密度脂蛋白胆固醇(HDL-C)水平高于治疗前,差异具有统计学意义(P<0.05)。MMT后患者月经周期延长、月经周期缩短、月经周期正常、闭经发生率与治疗前比较,差异无统计学意义(P>0.05)。对患者随访发现,生存质量改善改善率为50.00%(50例)。结论MMT能提高女性海洛因依赖患者性功能、BUN水平,降低Lp(a)水平,对女性海洛因依赖患者具有重要意义。Objective To investigate the effects of methadone maintenance therapy(MMT)on sexual function and levels of glutamic-pyruvic transaminase(GPT)and lipoprotein a[LP(a)]in female heroin addicats.Methods 100 female heroin addicts who visited Minzhu Road community drug maintenance clinic,Laodong community health clinic,Xujiapeng community drug maintenance clinic,Weimin health clinic and Wudong hospital psychiatric clinic from November 2018 to November 2019 were selected.Oral methadone maintenance therapy was given for 1 year.The female sexual function index(FSFI)questionnaire was used to evaluate the sexual function of the patients.The levels of GPT,BUN,Lp(a)and blood lipid were compared before and after treatment.The menstruation of the patients before and after treatment was observed and the life quality of the patients was evaluated.Results After MMT,the scores of sexual desire,sexual arousal and orgasm were lower than those before treatment,while the scores of vaginal lubrication,pain during intercourse,sexual satisfaction status and overall sexual function were higher than those before treatment,with statistical significance(P<0.05).There was no significant difference in GPT level after MMT compared with that before treatment(P>0.05).After treatment,BUN level was higher than before treatment,Lp(a)level was lower than before treatment,and HDL-C level was higher than before treatment,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of menstrual cycle lengthened,shortened,normal and amenorrhea after MMT compared with before treatment(P>0.05).Follow-up showed that the improvement rate of quality of life was 50.00%(50 cases).Conclusions MMT can improve the sexual function and BUN level of female heroin addicts,and reduce the Lp(a)level,which is of great significance for female heroin addicts.
