探讨TVS联合FIESTA对植入型凶险性前置胎盘的分析

摘　　要：目的探讨经阴道超声检查(TVS)联合快速稳态进动采集序列(FIESTA)对植入型凶险性前置胎盘的诊断分析价值。方法选取2016年3月至2018年6月西安市第四医院收治的110例疑似植入型凶险性前置胎盘孕妇作为研究对象。使用自身对照法,所有患者均接受TVS、FIESTA检查,并随访手术以及病理检查,金标准采用术后病理标准。比较TVS、FIESTA单一诊断与TVS联合FIESTA诊断的准确率、灵敏度以及特异度。结果术后病理检查结果提示,110例患者中植入型凶险性前置胎盘100例、非植入型凶险性前置胎盘10例;TVS联合FIESTA诊断的灵敏度99.00%、特异度90.00%、准确度98.18%,大于TVS的74.00%、60.00%、72.73%以及FIESTA的83.00%、70.00%、81.82%,差异具有统计学意义(P<0.05);TVS与FIESTA的灵敏度、特异度、准确度比较,差异无统计学意义(P>0.05)。结论在植入型凶险性前置胎盘诊断中,TVS联合FIESTA的诊断效能较单独应用TVS或FIESTA更优,能够满足临床诊疗所需,值得进一步推广使用。Objective To explore the diagnostic and analytical value of transvaginal ultrasonography(TVS)combined with fast imaging employing steady-state acquisition(FIESTA)for implantable dangerous placenta previa.Methods 110 pregnant women with suspected implantable dangerous placenta previa admitted to Xi′an Fourth Hospital from March 2016 to June 2018 were selected as the research subjects.All patients received TVS and FIESTA,followed up for surgery and pathological examination,and the gold standard was postoperative pathological examination.The accuracy,sensitivity and specificity of single diagnosis by TVS and FIESTA respectively and combined diagnosis of TVS and FIESTA were compared.Results Postoperative pathological examination showed that among 110 patients,there were 100 cases of implantable dangerous placenta previa and 10 cases of non-implantable dangerous placenta previa.The sensitivity,specificity and accuracy of TVS combined with FIESTA were 99.00%,90.00%and 98.18%,higher than those of TVS(74.00%,60.00%,72.73%)and FIESTA(83.00%,70.00%,81.82%),and the difference was statistically significant(P<0.05).There was no significant difference in the sensitivity,specificity and accuracy between TVS and FIESTA(P>0.05).Conclusions In the diagnosis of implantable dangerous placenta previa,the diagnostic efficacy of TVS combined with FIESTA is better than that of TVS or FIESTA alone,which can meet the needs of clinical diagnosis and treatment,and is worthy of further promotion.
