精准前列腺动脉栓塞术对高龄良性前列腺增生患者的疗效及相关实验室指标的影响

摘　　要：目的观察精准前列腺动脉栓塞术对高龄良性前列腺增生患者的疗效及相关实验室指标的影响。方法选取2015年1月至2018年11月在中国人民解放军联保部第980医院血管外科治疗的110例高龄良性前列腺增生患者作为研究对象。采用简单抽样法将其分为观察组(n=55)和对照组(n=55),观察组给予精准前列腺动脉栓塞术治疗,对照组给予经尿道前列腺切除术治疗。比较两组手术总有效率及手术时间、住院时间、导尿管留置时间;比较两组术前及术后3个月的男性性功能评价表(BMSFI)评分;比较两组术前及术后3个月的前列腺体积、排泄后残余尿量、最大尿流率;统计两组术后并发症及性功能障碍发生情况。结果观察组手术总有效率高于对照组,住院时间、导尿管留置时间短于对照组,并发症发生率低于对照组,差异具有统计学意义(P<0.05);术后3个月,观察组BMSFI评分高于对照组,排泄后残余尿量、前列腺体积小于对照组,最大尿流率高于对照组,差异具有统计学意义(P<0.05);两组手术时间、性功能障碍发生率比较,差异无统计学意义(P>0.05)。结论采用精准前列腺动脉栓塞术治疗高龄良性前列腺增生患者有效率高,能缩小前列腺体积,提高最大尿流率,降低并发症发生率。Objective To observe efficacy of precise prostate artery embolization(PAE)on the elderly with benign prostatic hyperplasia(BPH)and its influences on related laboratory indexes.Methods A total of 110 elderly patients with BPH treated in Department of Vascular Surgery,the 980th Hospital,Logistic Support Troop of Chinese PLA,from January 2015 to November 2018 were enrolled.They were divided into observation group(n=55)and control group(n=55)by simple random sampling method.The observation group was treated with preci-sion PAE,while control group was treated with transurethral resection of the prostate(TURP).The total surgical response rate,operation time,hospitalization time,catheter indwelling time,scores of brief male sexual function in-ventory(BMSFI)before surgery and after 3 months of surgery,residual urine volume,volume of prostate and maxi-mum flow rate before surgery and after 3 months of surgery were compared between the two groups.The occurrence of postoperative complications and sexual dysfunction in both groups was statistically analyzed.Results The total surgical response rate in observation group was higher than that in control group,hospitalization time and catheter indwelling time were shorter than those in control group,and incidence of complications was lower than that in con-trol group(P<0.05).After 3 months of surgery,BMSFI score in observation group was higher than that in control group,residual urine volume,volume of prostate after excretion was less than that in control group,and maximum flow rate was higher than that in control group(P<0.05).There was no significant difference in operation time or incidence of sexual dysfunction between the two groups(P>0.05).Conclusions Precise PAE is effective in trea-ting elderly BPH patients,which can reduce prostate volume,increase maximum flow rate and reduce incidence of complications.
