仙蓉合剂联合高强度超声聚焦治疗子宫腺肌病的疗效及对卵巢储备功能的影响

摘　　要：目的观察仙蓉合剂联合高强度超声聚焦(HIFU)治疗子宫腺肌病的疗效及对卵巢储备功能的影响。方法选取2017年10月至2019年5月贵州中医药大学第一附属医院诊治的90例子宫腺肌病患者作为研究对象。按随机数字表法分为A组(单纯HIFU治疗,n=30)、B组(HIFU联合米非司酮治疗,n=30)、C组(仙蓉合剂加减联合HIFU治疗,n=30)。记录三组治疗前后痛经评分、月经量、子宫体积、病灶体积及卵巢储备功能变化,并统计三组临床治疗总有效率。结果三组总有效率比较,差异具有统计学意义(χ~2=9.718,P<0.05);C组总有效率(93.33%)高于A组(63.33%),差异具有统计学意义(连续矫正χ~2=6.285,P<0.017)。治疗后,C组糖类抗原125(CA125)、黄体生成素(LH)、卵泡刺激素(FSH)、催乳素(PRL)、雌二醇(E_2)水平及痛经评分、月经量、子宫体积、病灶体积低于A组和B组,窦状卵泡计数(AFC)个数高于A组和B组,差异具有统计学意义(P<0.05);B组和A组CA125、LH、FSH、PRL、E_2水平及痛经评分、月经量、子宫体积、病灶体积比较,差异具有统计学意义(P<0.05)。结论仙蓉合剂联合HIFU治疗子宫腺肌病可改善卵巢储备功能和月经量,缩小子宫体积和病灶体积,减轻疼痛。Objective To observe the efficacy of Xianrong Mixture combined with high-intensity focused ultrasound(HIFU)in the treatment of uterine adenomyosis and its effect on ovarian reserve function.Methods A total of 90 patients with uterine adenomyosis admitted to the First Affiliated Hospital of Guizhou University of Tradi-tional Chinese Medicine from October 2017 to May 2019 were enrolled.According to random number table method,they were divided into group A(HIFU treatment alone,n=30),group B(HIFU combined with mifepristone treat-ment,n=30),and group C(Xianrong Mixture plus or minus HIFU treatment,n=30).Changes in dysmenorrhea score,menstrual volume,uterine volume,lesion volume and ovarian reserve function before and after treatment were recorded in the three groups,and the total clinical effective rate of the three groups was statistically analyzed.Results The total effective rate among the three groups was statistically significant(χ2=9.718,P<0.05).The total effective rate of group C(93.33%)was higher than that of group A(63.33%),and the difference was statis-tically significant(consecutive correctionχ2=6.285,P<0.017).Group C after treatment,carbohydrate antigen 125(CA125),luteinizing hormone(LH),follicle-stimulating hormone(FSH),prolactin(PRL),estradiol(E2)levels and dysmenorrhea score,menstrual quantity,uterine volume,lesion volume were lower than the group A and group B,AFC was higher than that of group A and group B,statistically significant difference(P<0.05).There were statistically significant differences in CA125,LH,FSH,PRL,E2 levels,dysmenorrhea score,menstrual vol-ume,uterine volume and lesion volume between group B and group A(P<0.05).Conclusions Xianrong Mixture combined with HIFU in the treatment of uterine adenomyosis can improve ovarian reserve function and menstrual vol-ume,reduce uterine volume and lesion volume,relieve pain.
