128排CT动态复查在创伤性颅脑损伤诊断中的临床价值

摘　　要：目的研究在创伤性颅脑损伤诊断中应用128排CT动态复查的价值。方法以2018年1月—2019年10月111例该院创伤性颅脑损伤患者为对象,均接受128排CT下入院后6 h内、入院后1 d内、入院后2~3 d 3个时间点的定时复查,分析检查结果。结果该组111例患者在入院后6 h内接受128排螺旋CT检查的诊断确诊率为61.26%,入院后1 d内诊断确诊率为76.74%,入院后2~3 d诊断确诊率为100.00%;CT检查征象显示111例患者中单纯蛛网膜下腔出血40例,单纯脑挫裂伤46例,同时合并有颅内血肿者25例;111例患者中轻型颅脑损伤有45例,中型颅脑损伤36例,重型颅脑损伤30例,不同CT计分级别恢复良好率、残疾率、病死率差异有统计学意义(P<0.05)。结论在创伤性颅脑损伤诊断中进行128排CT动态复查可获得较高确诊率,对患者预后有良好评估价值。Objective To study the value of 128-slice CT dynamic review in the diagnosis of traumatic brain injury.Methods From January 2018 to October 2019,111 patients with traumatic craniocerebral injury in our hospital were taken as subjects.They all received 128-slice CT within 6 hours after admission,within 1 day after admission,and 2-3 days after admission for regular review,analysis of inspection results.Results The diagnostic confirmation rate of 111 patients in this group who received 128-slice spiral CT within 6 hours after admission was 61.26%,the diagnostic confirmation rate within 1 day after admission was 76.74%,and the diagnostic confirmation rate was 100.00%2-3 days after admission;CT examination showed that among 111 patients,40 had simple subarachnoid hemorrhage,46 had simple cerebral contusion and laceration,and 25 had intracranial hematoma.Among 111 patients,45 had moderate and moderate craniocerebral injury.There were 36 cases of injury and 30 cases of severe head injury.There are statistically significant differences in the good recovery rate,disability rate,and mortality rate of different CT scoring levels(P<0.05).Conclusion In the diagnosis of traumatic brain injury,128-slice CT dynamic re-examination can obtain higher confirmation rate and has a good evaluation value for the prognosis of patients.
