摘 要:目的探讨上颌第一磨牙MB2根管再治疗采用显微超声技术联合CBCT检查的临床效果。方法对象选取为90例上颌第一磨牙MB2根管再治疗患者,研究起始时间为2016年10月—2018年10月,按照患者入院号,采用奇偶分配的方式将患者进行平分,其中抽取到偶数命名为对照组的45例患者采用普通X线片检查,配合显微超声技术疏通,抽取到奇数命名为观察组的45例患者采用CBCT检查,配合显微超声技术疏通,对比两组MB2发现率及根管治疗成功率,调查满意度及并发症。结果观察组MB2发现率及根管治疗成功率分别为88.89%及77.78%,与对照组71.11%及57.78%比较,显著更高,差异有统计学意义(χ^(2)=4.444、4.121,P<0.05);观察组总满意例数42例(93.33%),对照组总满意例数32例(71.11%),两组差异有统计学意义(χ^(2)=7.601,P<0.05)。患者在根管预备过程中未见侧壁穿孔、根尖偏移及器械分离等并发症,5个根管超填,3个根管欠填,其余均恰填。结论上颌第一磨牙MB2的发生率高,针对上颌第一磨牙MB2根管再治疗患者,采用显微超声技术联合CBCT检查能提高MB2根管发现率及根管治疗成功率,提高患者的满意度。Objective To investigate the clinical effect of retreatment of the MB2 root canal of the maxillary first molar with micro-ultrasound combined with CBCT.Methods The subjects were selected as 90 patients with maxillary first molar MB2 root canal retreatment.The study started in October 2016 and lasted until October 2018.The patients were divided equally according to the admission number of the patients and the odd-even allocation method.Forty-five patients with an even number named as the control group were examined by ordinary X-ray film,combined with micro-ultrasound technology,and 45 patients with an odd number named as the observation group were examined by CBCT,combined with micro-ultrasound technology,and the two groups were compared.MB2 present rate and root canal treatment success rate,survey satisfaction and complications.Results MB2 present rate and root canal treatment success rate of the observation group were 88.89%and 77.78%,which were significantly higher than those of the control group 71.11%and 57.78%,the difference was statistically significant(χ^(2)=4.444,4.121,P<0.05);the total number of satisfied people in the observation group was 42 case(93.33%),and the total number of satisfied people in the control group was 32 case(71.11%).There was statistically significant difference between the two groups(χ^(2)=7.607,P<0.05).During the root canal preparation,the patient had no complications such as side wall perforation,apical deviation,and instrument separation.5 root canals were overfilled,3 root canals were underfilled,and the rest were just filled.Conclusion The incidence of MB2 in the maxillary first molars is high.For patients undergoing MB2 root canal retreatment of the maxillary first molars,the use of micro-ultrasound technology combined with CBCT can improve the MB2 root canal discovery rate and the success rate of root canal treatment,and improve patient satisfaction.
