摘 要:目的系统评价托吡酯与氟哌啶醇治疗儿童抽动秽语综合征(Gilles de la Tourette syndrome, GTS)的有效性和安全性,为临床治疗提供参考依据。方法检索PubMed、Embase、中国知网、万方、维普等有关数据库,纳入评价托吡酯与氟哌啶醇治疗儿童GTS的随机对照试验(randomized controlled trial, RCT),检索时限从建库起至2020年9月。按纳入与排除标准筛选文献、提取资料并评价纳入研究的方法学质量后,采用Revman 5.4软件进行Meta分析。结果共7项RCTs纳入分析,包括464例患者,试验组242例,对照组222例。年龄2.5~18岁。试验组患者使用药物为托吡酯,对照组患者使用药物为氟哌啶醇,使用耶鲁综合抽动严重程度量表(Yale Global Tie Severity Scale, YGTSS)评价有效性。Meta分析结果显示,两组YGTSS的严重度总评分差异有统计学意义(MD=-17.64,95%CI:-19.06~-16.21,P<0.01);两组不良反应发生率差异有统计学意义(RR=0.51,95%CI:0.34~0.75,P<0.01)。结论托吡酯用于治疗GTS的有效性和安全性均优于氟哌啶醇。OBJECTIVE To systematically evaluate the efficacy and safety of topiramate versus haloperidol in the treatment of Gilles de la Tourette syndrome(GTS) in children, so as to provide references for the treatment of GTS. METHODS The databases of PubMed, Embase and CNKI were retrieved for randomized controlled trials(RCTS) evaluating topiramate versus haloperidol for TS in children starting from the establishment of the databases to September 2020. The Revman 5.4 software was used for Meta analysis after selecting the literature according to the inclusion and exclusion criteria, and extracting the data and evaluating the methodological quality of the included studie. RESULTS A total of 7 RCTs involving 464 patients of 2.5-18 years old were included with 242 patients in the experimental group administrating topiramate and 222 patients in the control group administrating haloperidol. The effectiveness was evaluated by the Yale Global Tie Severity Scale(YGTSS). Meta analysis results showed that the total severity scores of YGTSS in both groups were statistically significant(MD=-17.64, 95%CI:-19.06~-16.21, P<0.01). The total numbers of adverse reactions between both groups were statistically significant(RR=0.51, 95%CI: 0.34~0.75, P<0.01). CONCLUSION Topiramate is more effective and safe than haloperidol in the treatment of children with GTS.
