超声评估腹膜假黏液瘤的腹膜癌指数与肿瘤可切除的相关研究

摘　　要：目的探讨术前超声腹膜假黏液瘤(PMP)腹膜癌指数(PCI)与肿瘤可切除性的相关研究。方法术前对60例腹膜假黏液瘤患者进行超声检查,并进行超声腹膜癌指数(US-PCI)的计算,患者采用肿瘤细胞减灭术(CRS)治疗,术后进行肿瘤细胞减灭程度(CC)评分,分析术前US-PCI与肿瘤可切除的相关性。结果术前US-PCI评分与术后CC评分之间有显著相关性,术前US-PCI临界值为19.5时预测肿瘤可切除性的灵敏度和特异度最佳,当术前US-PCI<19.5时,预测PMP患者术后CC 0~1的可能性更大,当术前US-PCI≥19.5时,预测PMP患者术后CC 2~3的可能性更大。结论术前对腹膜假黏液瘤患者进行US-PCI评分,可以很好地预测肿瘤可切除性,对临床选择治疗方案及评估患者预后起到重要作用。Objective To explore the correlation between preoperative ultrasound peritoneal cancer index(PCI)and resectability of pseudomyxoma peritonei(PMP).Methods 60 PMP patients were examined by ultrasonography before operation,and the ultrasound peritoneal cancer index(US-PCI)was calculated.The patients were treated with cytoreductive surgery(CRS),the completeness of cytoreduction(CC)score was performed after the operation,and the correlation between preoperative US-PCI and tumor resectability was analyzed.Results There was a significant correlation between preoperative US-PCI score and postoperative CC score.The sensitivity and specificity of predicting tumor resectability was the best when the preoperative US-PCI critical value was 19.5.When the preoperative US-PCI<19.5,it was more likely to predict the postoperative CC 0-1 in PMP patients.When the preoperative US-PCI≥19.5,it was more likely to predict the postoperative CC 2-3 in PMP patients.Conclusions Preoperative US-PCI score for PMP patients can better predict the tumor resectability,and play an important role in the clinical selection of treatment options and the assessment of patient prognosis.
