小于胎龄儿与晚发型宫内生长受限胎儿右室心肌做功指数的研究

摘　　要：目的比较小于胎龄儿(SGA)与晚发型宫内生长受限(LO-IUGR)胎儿右室心肌做功指数的差异性。方法选取我院产检发现的晚发型宫内生长受限胎儿50例(孕32~37周)及同孕龄的小于胎龄儿50例,并选取孕龄相匹配的50例正常胎儿作为对照组,运用组织多普勒分别检测三组胎儿的右室心肌做功指数(MPI)。结果与对照组相比,晚发型宫内生长受限胎儿和小于胎龄儿的等容收缩期时间(ICT)及等容舒张期时间(IRT)显著延长(P<0.001),射血期时间(ET)明显缩短(P<0.001),心肌做功指数(MPI)显著增加(P<0.001)。晚发型宫内生长受限胎儿与小于胎龄儿相比,MPI存在差异性(P<0.001)。结论使用MPI可以评估胎儿的心功能变化,同时协助诊断和发现真正的宫内生长受限胎儿,为临床管理这些胎儿以及改善其妊娠结局提供依据。Objective To compare the difference of right ventricular myocardial performance index(MPI) between small-for-gestational age(SGA) and late-onset intrauterine growth restriction(LO-IUGR). Methods 50 cases of LO-IUGR fetuses(32 to 37 weeks of gestation) and 50 cases of SGA age were selected in our hospital. 50 normal fetuses with matched gestational ages were used as the control group. The right ventricular myocardial pevformance index(MPI) of the three groups of fetuses was measured using tissue Doppler. Results Compared with the control group, the isovolumetric contraction time(ICT) and isovolumetric relaxation time(IRT) of LO-IUGR and SGA were significantly prolonged(P<0.001) and the ejection period(ET) was significantly shortened(P<0.001), and MPI was significantly increased(P<0.001). There was a difference in MPI between LO-IUGR and SGA(P<0.001). Conclusions The use of MPI can assess fetal cardiac function changes, and at the same time assist in the diagnosis and discovery of true intrauterine growth-restricted fetuses, providing a basis for clinical management of these fetuses and improvement of their pregnancy outcomes.
