腹部外伤手术患者自体血液回收中不同浓度肝素抗凝剂的应用

摘　　要：目的腹部外伤手术患者自体血液回收中不同浓度肝素抗凝剂的应用价值分析。方法以2019年1月—2020年1月期间于该院接受腹部外伤手术治疗且使用自体血液回收技术的78例患者作为该次研究对象,按照治疗期间使用肝素抗凝剂浓度的差异性划分为a、b两组,每组39例;其中a组肝素抗凝剂浓度为12500 U肝素钠混合500 mL浓度为0.9%的生理盐水,b组肝素抗凝剂浓度为25000 U肝素钠混合500 mL浓度为0.9%的生理盐水,比对分析两组患者术中失血量、自体血回输量以及术前、自体血回输前后血指标改变情况。结果a、b两组患者的术中失血量、自体血回输量基本等同,a组分别为(2605.25±29.58)mL、(1524.76±26.05)mL,b组分别为(2611.49±22.68)、(1521.34±25.85)mL,即数据组间差异无统计学意义(t=1.045、0.582,P=0.299、0.562);组内自体血回输前HCT与Hb低于术前,自体血回输后与术前趋同,自体血回输后PLT低于术前,差异有统计学意义(P<0.05),其他指标趋同,差异无统计学意义(P>0.05);两组自体血回输后数据对比,a组HCT、Hb、APTT、PLT均稍高于b组,PT稍低于b组,差异无统计学意义(P>0.05)。结论相对比25000 U肝素钠而言,使用12500 U肝素钠作为抗凝剂在自体血回输中的应用价值更为显著,可以为患者提供较好的安全保障。Objective To analyze the application value of different concentrations of heparin anticoagulant in autologous blood recovery of patients with abdominal trauma.Methods 78 patients who underwent abdominal trauma surgery and used autologous blood recovery technology in the hospital from January 2019 to January 2020 were selected as the study objects,and were divided according to the difference in the concentration of heparin anticoagulant during the treatment period:groups a and b with 39 patients in each group;among them,the heparin anticoagulant concentration of group a is 12500 U heparin sodium mixed with 500 mL of 0.9%normal saline,and the heparin anticoagulant concentration of group b is 25000 U heparin sodium mixed with 500 mL concentration was 0.9%normal saline.The blood loss during the operation,the amount of autologous blood transfusion and the changes of blood indexes before and after the autologous blood transfusion were compared and analyzed between the two groups.Results The intraoperative blood loss and autologous blood transfusion of patients in groups a and b were basically the same.Group a was(2605.25±29.58)mL and(1524.76±26.05)mL,and group b was(2611.49±22.68)mL and(1521.34±25.85)mL,the difference between the data groups was not statistically significant(t=1.045,0.582,P=0.299,0.562);the HCT and Hb of the autologous blood before retransfusion in the group are lower than those before the operation,after autologous blood transfusion,it is the same as before operation,PLT after autologous blood reinfusion was lower than before operation,the difference was statistically significant(P<0.05),and other indicators were the same,the difference was not statistically significant(P>0.05);comparison of data after autologous blood reinfusion between the two groups,group a HCT,Hb,APTT Both PLT and PLT were slightly higher than in group b,and PT was slightly lower than in group b.The difference was not statistically significant(P>0.05).Conclusion Compared with 25000U heparin sodium,the application value of 1250
