摘  要:目的分析探究糖化血红蛋白测定在糖尿病诊断的临床意义。方法将2018年4月—2019年4月进入该院进行体检的136名健康人群划分为对照组,将同期该院收治的136例糖尿病患者划分为观察组,两组均实行免疫抑制投射比浊法测定糖化血红蛋白(HbA1c)指标水平,利用葡萄糖氧化酶法测定其空腹血糖(FPG)水平,进一步比较两组诊断结果。结果①观察组FPG、HbA1c均明显高于对照组,两组数据差异有统计学意义(P<0.05)。②根据FPG水平将观察组划分为3个等级,即:6~8 mmol/L、8 mmol/L<FPG<10 mmol/L、≥10 mmol/L,发现FPG水平与HbA1c间呈正相关性(r=0.916,P<0.05)。③根据HbA1c水平将观察组划为两个等级,即:<8%、≥8%;结果显示:当HbA1c水平≥8%时,周围神经病变、高血压、脑血管疾病、糖尿病肾病、视网膜病变并发症的发生率明显高于<8%等级,两组数据差异有统计学意义(P<0.05)。结论糖化血红蛋白测定能反映人体的血糖指标水平,对糖尿病诊断及病情跟踪具有非常重要的临床意义,因此值得采纳及应用。Objective To analyze and explore the clinical significance of glycosylated hemoglobin in the diagnosis of diabetes.Methods 136 healthy people who entered the hospital for physical examination from April 2018 to April 2019 were divided into the control group,and the 136 diabetic patients admitted to the hospital during the same period were divided into the observation group.Both groups were implemented the immunosuppressive projection turbidimetric method to determine the index level of glycosylated hemoglobin(HbA1c),and the fasting blood glucose(FPG)level was determined by the glucose oxidase method to further compare the diagnosis results of the two groups.Results 1.FPG and HbA1c in the observation group were significantly higher than those in the control group,and the difference between the two groups was statistically significant(P<0.05).2.According to the FPG level,the observation group was divided into 3 levels,namely:6~8 mmol/L,8 mmol/L<FPG<10 mmol/L,≥10 mmol/L,and it was found that there was a positive correlation between FPG level and HbA1c(r=0.916,P<0.05).3.According to the HbA1c level,the observation group was divided into two levels,namely:<8%,≥8%;the results showed:when the HbA1c level≥8%,peripheral neuropathy,hypertension,cerebrovascular disease,diabetic nephropathy,The incidence of complications of retinopathy was significantly higher than the level of<8%,and the difference between the two groups of data was statistically significant(P<0.05).Conclusion The determination of glycosylated hemoglobin can reflect the blood glucose index level of the human body,and has very important clinical significance for the diagnosis of diabetes and disease tracking,so it is worthy of adoption and application.
