盐酸伊伐布雷定对高龄老年冠心病合并心功能不全患者心率及心功能的影响

摘　　要：目的分析盐酸伊伐布雷定对高龄老年冠心病合并心功能不全患者心率及心功能的影响。方法选择我院2015年1月至2020年12月收治的50例高龄老年冠心病合并心功能不全患者,将其中应用伊伐布雷定治疗的25例患者作为观察组,采用倾向性评分匹配法得到未应用伊伐布雷定治疗的25例患者作为对照组。比较两组患者的心功能指标改善情况及预后效果。结果观察组心功能改善2级时间、住院时间短于对照组,住院总费用少于对照组(P<0.05)。治疗后,两组心率、血浆脑钠肽水平、左心室射血分数、左室舒张末期内径均较治疗前改善,且观察组患者心率、血浆脑钠肽水平、左室舒张末期内径均低于对照组(P<0.05)。两组均无严重不良反应。结论常规药物联合盐酸伊伐布雷定治疗高龄老年冠心病合并心功能不全患者的效果确切,有助于改善患者的心功能。Objective To analyze the effect of ivabradine hydrochloride on the heart rate and heart function of elderly patients with coronary heart disease and cardiac insufficiency.Methods We selected 50 elderly patients with coronary heart disease and cardiac insufficiency who were admitted to our hospital from January 2015 to December 2020.Among them,25 patients treated with ivabradine were selected as the observation group,and the propensity score matching method was used to obtain the results.Twenty-five patients treated with ivabradine served as a control group.The improvement of cardiac function indexes and prognostic effects of the two groups of patients were compared.Results The observation group's cardiac function improved by grade 2,the length of hospital stay was shorter than that of the control group,and the total hospitalization cost was less than that of the control group(P<0.05).After treatment,the heart rate,plasma brain natriuretic peptide level,left ventricular ejection fraction,and left ventricular end-diastolic diameter were improved in the two groups compared with before treatment.The observation group's heart rate,plasma brain natriuretic peptide level,and left ventricular end-diastolic diameter were all lower than before treatment and the control group(P<0.05).There were no serious adverse reactions in both groups.Conclusion Conventional drugs combined with ivabradine hydrochloride have a definite effect in the treatment of elderly patients with coronary heart disease and cardiac insufficiency,and help to improve the cardiac function of patients.
