温针疗法对颞下颌关节紊乱综合征患者fricton指数的影响

摘　　要：目的探讨温针疗法对颞下颌关节紊乱综合征患者颞下关节功能指数的影响。方法选取符合纳入标准的60例作为研究对象,根据随机数字表分为治疗组和对照组。对照组给予双氯芬酸二乙胺乳胶剂(扶他林)外用,治疗组用温针灸治疗,观察治疗前后颞下颌关节功能指数(Fricton指数),连续治疗2个疗程。结果2组治疗前下颌运动分(MN)、关节杂音分(JN)、关节压诊分(JP)、功能障碍指数(DI)、肌肉压诊分(MP)、肌肉压痛指数(PI)、颞下颌关节指数(CMI)比较,差异无统计学意义(P>0.05)。治疗后2组MN、JN、JP、DI、MP、PI、CMI评分均低于治疗前,且治疗组所有评分前后差异有统计意义(P<0.05),对照组JN评分治疗前后差异无统计意义(P>0.05);2组治疗后差值比较,JP评分差异无统计学意义(P>0.05),其余项治疗组优于对照组(P<0.05);治疗组总有效率92.86%,高于对照组82.76%,两者疗效差异有统计学意义(P<0.05)。结论温针灸治疗颞下颌关节紊乱综合征,能显著改善患者的功能障碍和疼痛症状,提高疗效,且优于西药组。Objective:To explore the effect of warm acupuncture therapy on the functional index of the infra temporomandibular joint in patients with temporomandibular joint disorder syndrome.Methods:60 patients that met the inclusion criteria were selected as the research objects and randomly divided into a treatment group and a control group.The control group was given diclofenac diethylamine latex(voltaren)for external use and the treatment group was treated with warm acupuncture.The temporomandibular joint function index(Fricton index)was observed before and after treatment,and the treatment continued for 2 courses.Results:Before treatment,the mandibular movement score(MN),joint noise score(JN),joint pressure score(JP),dysfunction index(DI),muscle pressure score(MP),muscle pain index(PI)and temporomandibular joint index(CMI)had no statistically significant difference(P>0.05).After treatment,the scores of MN,JN,JP,DI,MP,PI,and CMI in the two groups were lower than those before treatment.The differences of all the scores in the treatment group before and after treatment were statistically significant(P<0.05),and the differences of the JN scores in the control group before and after treatment were not statistically significant(P>0.05).In comparison of the difference between the two groups after treatment,the difference of the JP score was not statistically significant(P>0.05),and the rest of the treatment group was better than that of the control group(P<0.05).The total effective rate of the treatment group was 92.86%,higher than 82.76%of the control group.The difference in the curative effect between the two groups was statistically significant(P<0.05).Conclusion:Warm acupuncture can treat temporomandibular joint disorder syndrome and significantly improve the dysfunction and pain symptoms of the patients,and has a better effect than the western medicine.
