透明质酸钠注射导致眶周血管栓塞的临床治疗效果观察

摘　　要：目的探讨透明质酸钠注射导致眶周血管栓塞引发视力损害及皮肤软组织损伤的紧急对症处置及后续治疗效果。方法自2013年1月至2021年1月辽宁省人民医院整形科对收治的18例面部注射透明质酸钠导致眶周血管栓塞的患者给予综合治疗,包括透明质酸酶注射、扩血管、改善局部组织微循环、抗感染、营养神经、高压氧舱、局部软组织清创换药等。结果本组共18例患者,均为透明质酸钠注射术后血管栓塞伴有不同程度的视力损伤、眼肌麻痹及皮肤软组织坏死,通过综合治疗后效果较满意。结论对于透明质酸钠面部注射引起的眶周血管栓塞,采取及时、积极的对症治疗后,可有效地缓解减轻眼部视力损害、眼肌麻痹和皮肤软组织坏死等并发症,可获得良好的愈后效果。Objective To explore the emergency treatment and follow-up treatment of visual impairment and skin and soft tissue necrosis caused by periorbital vascular embolism after sodium hyaluronate gel injection.Methods From January 2013 to January 2021,18 patients with periorbital vascular embolism after sodium hyaluronate gel injection underwent the comprehensive treatment.The treatment methods including hyaluronidase injection,vascular dilatation,improvement of local tissue microcirculation,anti-infection,neurotrophic treatment,hyperbaric oxygen chamber,debridement and dressing change of local soft tissue.Results The vascular embolism with varying degrees of visual impairment,ophthalmoplegia and skin and soft tissue necrosis after sodium hyaluronate gel injection occurred in 18 patients.All patients were satisfited with the efficacy of comprehensive treatment.Conclusion For periorbital vascular embolism caused by sodium hyaluronate gel injection,timely and active symptomatic treatment can effectively alleviate the complications such as visual impairment,ophthalmoplegia and skin and soft tissue necrosis.Good recovery efficacy can be obtained.
