加味四妙汤辅助治疗痛风性关节炎对RAGE、COX-2水平变化研究

摘　　要：目的研究加味四妙汤辅助西医治疗痛风性关节炎对患者糖基化终产物受体(RAGE)、环氧合酶-2(COX-2)水平变化影响。方法选取医院2017年1月—2019年1月收治的痛风性关节炎患者76例,采用随机数字法分为两组。对照组给予西医治疗,观察组给予加味四妙汤辅助西医治疗,分析两组患者治疗后的临床效果。结果观察组总有效率(94.74%,36/38)高于对照组(78.95%,30/38),总不良反应率(31.58%,12/38)与对照组(21.05%,8/38)比较,差异不具有统计学意义(P>0.05)。两组治疗前视觉模拟(VAS)评分组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后VAS评分(2.47±0.24)分低于对照组(P<0.05)。两组治疗前RAGE、COX-2、血沉(ESR)、炎症因子水平组间比较,差异不具有统计学意义(P>0.05)。观察组治疗后RAGE、COX-2、ESR、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)水平低于对照组(P<0.05)。结论加味四妙汤辅助西医治疗痛风性关节炎可减轻炎症反应和疼痛程度,降低RAGE、COX-2、ESR的表达水平,提高疗效,且不增加不良反应。Objective To study the effect of Jiawei Simiao Decoction(加味四妙汤) on the levels of glycosylation end product receptor(RAGE) and cyclooxygenase-2(COX-2) in the treatment of gouty arthritis. Methods Seventy-six patients with gouty arthritis treated in our hospital from January 2017 to January 2019 were randomly divided into two groups. The control group was treated with western medicine, and the observation group was treated with Jiawei Simiao Decoction. The clinical effect of the two groups was analyzed. Results The total effective rate(94.74%) in the observation group was higher than that in the control group(78.95%), and the total adverse reaction rate(31.58%) was not significantly different from that in the control group(21.05%). There was no significant difference in visual simulation(VAS) score between the two groups before treatment(P>0.05). After treatment, the score of VAS in the observation group(2.47±0.24) was lower than that in the control group(P<0.05). There was no significant difference in the levels of ESR, inflammatory factors,RAGE,COX-2 and erythrocyte sedimentation rate between the two groups before treatment(P>0.05). After treatment, the levels of RAGE,COX-2,ESR, TNF-α, IL-6 and IL-8 in the observation group were lower than those in the control group(P<0.05). Conclusion Jiawei Simao Decoction combined with western medicine in the treatment of gouty arthritis can reduce the degree of inflammation and pain, decrease the levels of RAGE,COX-2 and ESR, improve the curative effect, and do not increase the adverse reactions.
