基于中医学认识防治糖尿病视网膜病变的新药发现思路

摘　　要：糖尿病视网膜病变(diabetic retinopathy,DR)是一种严重的糖尿病(DM)并发症,在中医学中糖尿病属于"消渴"的范畴,现代中医家称呼糖尿病视网膜病变为"消渴目病"。基于中医的整体观念和辨证论治对"消渴"和"消渴目病"的独特认识,认为"消渴目病"的病机是阴虚燥热、气虚血瘀。基于中医古籍《黄帝内经》《千金方》等对"消渴"和"消渴目病"的认识和治疗,挖掘古典临床治疗"消渴目病"的常用中药及中药方剂,发现了黄芪、丹参、地黄在古籍中治疗"消渴目病"效果显著,且现代研究表明黄芪多糖、黄芪甲苷等有效成分可以降低VEGF的水平来抑制视网膜炎症。以期为DR的治疗及新药发现提供新思路,进一步发挥中医药的特色优势。Diabetic retinopathy(DR) is a serious complication of diabetes mellitus(DM). In traditional Chinese medicine, diabetes belongs to "Xiaoke". Modern Chinese physicians call DR "Xiaoke eye disease". Based on the overall concept of TCM and the unique understanding of "Xiaoke" and "Xiaoke eye disease", this paper holds that the pathogenesis of "Xiaoke eye disease" is Yin deficiency, dryness and heat, Qi deficiency and blood stasis. Based on the understanding and treatment of "Xiaoke" and "Xiaoke eye disease" in Huangdi Neijing and Qianjin Fang, and excavating the traditional Chinese medicine and prescriptions commonly used in the classical clinical treatment of "Xiaoke eye disease", we have found that Huangqi(Astragalus membranaceus), Danshen(Salvia miltiorrhiza) and Dihuang(Rehmannia) have remarkable effect on treating "Xiaoke eye disease" in ancient books, and modern research shows that Astragalus polysaccharides and astragaloside IV can reduce the level of VEGF to inhibit retinitis inflammation. The purpose of this paper is to provide new ideas for the treatment of DR and the discovery of new drugs, and further develop the characteristic advantages of traditional Chinese medicine.
