临床药师对1例建立家庭病床脑梗死后遗症患者的药学服务

摘　　要：本文选取临床药师参与的1个因脑梗死后遗症长期卧床的家庭病床患者治疗的案例,通过对该患者不同疾病及对应的治疗药物进行分析及建议,并对治疗过程中出现的不良反应进行监护和处理,改善患者疾病症状、提高生活质量且节省了药品费用,保障患者用药安全、有效、经济,从而为建立"4+X"家庭医生团队服务模式、开展居家药学服务提供依据。This study selected long-term treatment of one patient with cerebral infarction sequelae in family beds participated by clinical pharmacists as the case.Through the analysis and recommendations on the treatment of the patient with different diseases and the corresponding drug uses,and monitoring and processing of the adverse drug reactions during the treatment,the patient’s symptoms were alleviated,the quality of life was improved,the drug cost was saved,and the safety,effectiveness and economic of drug use were ensured.The results provide basis for the establishment of "4+X" family doctor team service pattern and the performance of home pharmaceutical care.
