益气活血化瘀汤联合针灸治疗缺血性脑卒中偏瘫的疗效观察

摘　　要：目的探讨益气活血化瘀汤联合针灸治疗缺血性脑卒中偏瘫的临床疗效。方法将医院2016年2月—2019年4月收治的缺血性脑卒中偏瘫患者84例按随机数字表法随机分为观察组和对照组各42例。对照组行针灸治疗,失语穴取风池、廉泉等;腰部无力穴取肾俞、足三里;上肢偏瘫穴取外关、合谷、曲池等;下肢偏瘫者穴取绝骨、环跳、阴陵泉、昆仑、足三里等,平补平泻法,留针20 min,1次/d,治疗5 d,休息2 d,连续治疗1个月。观察组在此基础上加用益气活血化瘀汤,水煎400 mL,早晚分服,1剂/d,治疗1个月。比较两组临床疗效、肢体运动功能、生活质量。结果治疗后,观察组总有效率95.24%(40/42)高于对照组78.57%(33/42),差异有统计学意义(P<0.05).肢体运动功能(Fugl-Meyer量表)、生活质量(SF-36评分)两组均有提高,观察组高于对照组,差异均有统计学意义(P<0.05)。结论活血化瘀汤联合针灸治疗缺血性脑卒中偏瘫疗效确切,可有效改善肢体运动功能,提高生活质量,有助于患者康复,值得推广。Objective To explore the clinical effect of Yiqi Huoxue Huayu decoction combined with acupuncture on hemiplegia after ischemic stroke.Methods 84 hemiplegic patients with ischemic stroke admitted in our hospital from February 2016 to April 2019 were randomly divided into observation group and control group with 42 cases in each group according to the random number table method.The control group was treated with acupuncture and moxibustion,the aphasia patients were treated with Fengchi,Lianquan,Shenshu,Zusanli,Waiguan,Hegu,Quchi,etc.the upper hemiplegia patients were treated with juebun,Huantiao,yinlingquan,Kunlun,Zusanli,etc.the treatment group was treated with pingbu pingxie,retaining needles for 20 minutes,once a day,5 days of treatment,2 days of rest and 1 month of continuous treatment.On this basis,the observation group was added with Yiqi Huoxue Huayu decoction,decoction 400 ml,take it separately in the morning and evening,one dose per day,for one month.The clinical effect,limb motor function and quality of life of the two groups were compared.Results After treatment,the total effective rate of the observation group was 95.24%(40/42)higher than that of the control group 78.57%(33/42),the difference was statistically significant(P<0.05).Conclusion The therapeutic effect of Yiqi Huoxue Huayu decoction combined with acupuncture and moxibustion on hemiplegia after ischemic stroke is definite,which can effectively improve the motor function of limbs,improve the quality of life,help the patients recover,and is worth popularizing.
