升降抑幽汤与奥美拉唑联合对Hp相关出血性胃炎疗效及胃黏膜改变情况的影响

摘　　要：目的研究升降抑幽汤与奥美拉唑联合用药对幽门螺杆菌(helicobacter pylori,Hp)相关出血性胃炎疗效及胃黏膜改变情况的影响。方法以2017年1月-2020年1月邢台医学高等专科学校第二附属医院消化内科收治的122例Hp相关出血性胃炎患者为研究对象,采用随机数字表法分为观察组和对照组各61例。对照组服用奥美拉唑治疗,观察组在对照组基础上联合服用升降抑幽汤治疗,连续服用30 d。比较中医症状积分、止血时间以及充血、水肿、糜烂等出血点胃黏膜改变情况、Hp根除率变化情况;另采用酶联免疫吸附测定法(ELISA)法检测两组患者的血清胃泌素-17、胃蛋白酶原Ⅰ(PGⅠ)、胃蛋白酶Ⅱ(PGⅡ)表达水平。结果治疗30 d后,与对照组比较,观察组治疗有效率和Hp转阴率升高,症状和病理积分降低,血清胃泌素-17水平升高,PGⅠ和PGⅡ水平降低;观察组止血时间和腹部灼痛、黑便、食欲减退、呕血、恶心等临床症状消失时间缩短,差异有统计学意义(P<0.05)。结论升降抑幽汤与奥美拉唑治疗Hp相关出血性胃炎临床疗效显著,适合临床推广应用。Objective To investigate the effect of the combination of Shengjiang Yiyou Decoction and omeprazole on the curative effect of Helicobacter pylori(Hp)-related hemorrhagic gastritis and gastric mucosal changes.Methods A total of 122 patients with Hp-related hemorrhagic gastritis who were admitted to the Department of Gastroenterology of the Hospital from January 2017 to January 2020 were selected as the research objects,and were randomly divided into observation group and control group with 61 cases for each.The control group was treated with omeprazole,and the observation group was treated with Shengjiang Yiyou Decoction on the basis of the control group for 30 consecutive days to compare the TCM symptom score,hemostasis time,and changes in the gastric mucosa of bleeding points such as congestion,edema,and erosion,Hp eradication rate changes;In addition,enzyme-linked immunosorbent assay(ELISA)was used to detect the expression levels of serum gastrin-17,pepsinogenⅠ(PGⅠ),and pepsinⅡ(PGⅡ)in the two groups.Results After 30 days of treatment,compared with the control group,the effective rate and HP negative conversion rate of the observation group increased,the symptom and pathological score decreased,the serum gastrin-17 level increased,and the PGⅠand PGⅡlevels decreased;the hemostatic time and the disappearance time of clinical symptoms such as abdominal burning pain,melena,anorexia,hematemesis and nausea in the observation group were shortened(P<0.05).Conclusion Shengjiang Yiyou Decoction and omeprazole have significant clinical effect in the treatment of Hp-related hemorrhagic gastritis,which is suitable for clinical application.
