清肠通便胶囊对功能性便秘的临床疗效及其干预肠道菌群的作用机制

摘　　要：目的:观察清肠通便胶囊对功能性便秘的临床疗效并探讨其作用机制。方法:选取2019年2月至12月北京丰台右安门医院功能性便秘患者61例,随机分为治疗组和对照组。治疗组给予口服清肠通便胶囊,对照组给予乳果糖口服液,疗程2周。停药2周后连续7d观察并比较两组临床疗效、临床症状积分和中医症状评分。并于治疗前后通过高通量测序分析清肠通便胶囊对肠道菌群的影响。选取30只雄性SPF级CD-1小鼠,分为实验组和对照组,每组15只,实验组予以清肠通便胶囊混悬液灌胃治疗,对照组予以去离子水,30min后计算两组小鼠墨汁推进百分率,观察肠蠕动情况。结果:经清肠通便胶囊治疗后,临床总有效率为86.67%(26/30)显著高于对照组(19/31)(P<0.05);治疗组治疗后的临床症状积分和中医症状评分均显著优于治疗前(P<0.05);清肠通便胶囊显著增加了墨汁在小鼠肠道内推进长度(P<0.01);此外清肠通便胶囊能够增加肠道丁酸盐菌及芽孢杆菌。结论:清肠通便胶囊可通过促进肠蠕动及正向调节肠道菌群,对功能性便秘患者发挥良好治疗效果。Objective: To observe the clinical effect of Qingchang Tongbian Capsule on functional constipation and explore its mechanism. Methods: A total of 61 cases of functional constipation were selected in Beijing Fengtai YouAnMen Hospital from February to December 2019, which were randomly divided into a treatment group and a control group. The patients of treatment group were treated with Qingchang Tongbian Capsule for 2 weeks, while the patients of control group were given Lactulose Oral Liquid for 2 weeks. After stopping drug for 2 weeks, the clinical effect and TCM symptom scores of two groups were compared during seven consecutive days, meanwhile, the effect of Qingchang Tongbian Capsule on intestinal flora through high throughput sequencing was analyzed. Thirty male SPF CD-1 mice were selected,which were divided into an experimental group and a control group, 15 in each group. The experimental group were feeding by mixed suspension of Qingchang Tongbian Capsule, and the control group were feeding by deionized water. The peristalses were observed according to the movement of ink after 30 min. Results: The total effective rate was 86.67%(26/30), which was superior to that in the control group(19/31)(P<0.05) and the clinical symptom score and TCM symptom score after treatment were significantly superior to those before treatment(P<0.05). The Chinese ink propulsion length in the intestine of mice was significantly increased by Qingchang Tongbian Capsule(P<0.01). In addition, Qingchang Tongbian Capsule could promote the increase of intestinal butyrate bacteria and bacillus. Conclusion: Qingchang Tongbian Capsule has a good therapeutic effect on patients with functional constipation by promoting intestinal peristalsis and positively regulating intestinal flora.
