摘　　要：目的探讨白细胞介素-21(IL-21)、白细胞介素-17(IL-17)在IgA肾病(IgAN)的表达与肿瘤坏死因子-α(TNF-α)、转化生长因子-β(TGF-β)及病理分级的关系。方法纳入73例IgAN患者为IgAN组,纳入既往无任何疾病因肾外伤行肾切除患者40例为正常对照组,分别收集2组血清和肾组织标本。用ELISA方法检测2组血清IL-21、IL-17的表达;用RT-PCR方法检测2组肾组织IL-21、IL-17、TNF-α、TGF-β的表达,并分析IL-21、IL-17表达和TNF-α、TGF-β之间的相关性;用免疫组化方法检测2组肾组织IL-21、IL-17、TNF-α的表达定位,并分析其表达与不同病理分级表达强度之间的关系。结果IgAN组血清IL-21、IL-17的表达较对照组明显升高,差异有统计学意义(P<0.05)。IgAN患者肾组织中IL-21、IL-17、TNF-α、TGF-β表达较对照组均明显升高,差异有统计学意义(P<0.05),且IL-21、IL-17表达与TNF-α呈正相关,与TGF-β没有相关性。IL-21、IL-17、TNF-α在IgAN患者肾小管上皮细胞呈阳性表达,并随病理分级加重,表达强度随之增加。结论IgAN患者IL-21、IL-17高表达,且与炎症因子TNF-α表达和病理分级存在正相关,有望成为评估IgAN早期炎症活动的生物标志物。Objective To investigate the expression of interleukin-21(IL-21)and interleukin-17(IL-17)in IgA nephropathy(IgAN)and its relationship with tumor necrosis factor-α(TNF-α),transforming growth factor-β(TGF-β)and pathological grade.Methods A total of 113 patients were enrolled this study,73 IgAN patients were included in the IgAN group,and 40 patients with nephrectomy due to renal trauma without any previous disease were included in the control group.The serum samples and kidney tissues of the two groups were respectively collected and the concentration of serum IL-21 and IL-17 was detected in two groups.RT-PCR was used to detect the expression of IL-21,IL-17,TNF-αand TGF-βin renal tissue of the two groups,and the correlation between the expression of IL-21,IL-17 in IgAN and TNF-α,TGF-βwere analyzed.The expression sites of IL-21,IL-17 and TNF-αin renal tissues were detected by immunohistochemical method,and the relationship between the expression levels and the expression intensity of different pathological grades was analyzed.Results The expression of IL-21 and IL-17 of IgAN group was obviously higher than those of control group(P<0.05).The expression of IL-21,IL-17,TNF-α,TGF-βin renal tissue of IgAN group were significantly higher than those in control group(P<0.05).The expression of IL-21 and IL-17 was positively correlated with TNF-α.IL-21,IL-17 and TNF-αwere positively expressed in renal tubular epithelial cells of patients with IgAN,and the expression intensity increased with the progression of pathological grade.Conclusion IL-21 and IL-17 are highly expressed in IgAN patients,and their expression are positive correlated with inflammatory factors TNF-αand pathological grade,which may be used as biomarkers to evaluate the early inflammatory activity of IgAN.
